Zusammenfassung
Der M. latissimus dorsi verläuft von der Fascia thoracolumbalis der Lenden- und unteren Brustwirbelsäule direkt unter der Haut zu seinem Ansatz am Oberarm (Crista tuberculi minoris). Die 1906 von Tansini erstbeschriebene M. latissimus dorsi Lappenplastik war nach der Wiederentdeckung durch Olivari 1976 aufgrund der sehr konstanten Anatomie schnell zur Standardlappenplastik geworden. Er kann als freier Lappen oder als gestielter Lappen verwendet werden. Im Rahmen der Brustchirurgie wird er fast nur gestielt verwendet. Noch heute ist die gestielte Latissimus, LADO oder Lati genannte Lappenplastik ein gutes Verfahren, wenn die freien Lappenplastiken nicht indiziert oder gescheitert sind. Der Latissimus ist ein Innenrotator und Adduktor in der Schulter. Bei intakter Schulter und vorhandener restlicher Muskulatur ist die Muskelentnahme meist gut toleriert.
