Zusammenfassung
Die Neuroosteoarthropathie (NOAP) von Fuß und Sprunggelenk wird durch ossäre Destruktionen mit Subluxation, Luxation und/oder pathologische Frakturen charakterisiert, die zu erheblichen Instabilitäten, Deformitäten und Komplikationen führen können. Ein Spätstadium der NOAP am Rückfuß ist kaum zu verkennen. Die Dynamik der Erkrankung mit zunehmender Häufung von Komplikationen bei verzögerter Diagnosestellung erfordert es, eine möglichst frühe Diagnosestellung zu erzielen. Bei peritalarer Manifestation der DNOAP ist eine Rückfußarthrodese unter Einschluss von OSG und Subtalargelenk das Verfahren der Wahl. Bei höhergradigen Defekten sollte subtraktiven Techniken gegenüber jenen, die auf Rekonstruktion der ursprünglichen Beinlänge abzielen, bevorzugt werden. In der Literatur wurden hohe Komplikationsraten berichtet, die sogar die 40 %-Marke überschreiten. Diese Zahlen kontrastieren mit einer hohen Patientenzufriedenheit von 80 % und einem Extremitätenerhalt von 97 %.
