Zusammenfassung
Im letzten Jahrzehnt hat sich die Technik der endovaskulären Aortenaneurysmareparation (EVAR) als Alternative zu der bisherigen offenen chirurgischen Therapie herauskristallisiert. Randomisierte Studien wie DREAM und EVAR1 konnten im Vergleich zur offenen chirurgischen Therapie deutliche Vorteile aufseiten der endovaskulären Methode aufzeigen (Greenhalgh et al. 2004; Prinssen et al. 2004). Auch wenn die mittelfristigen Ergebnisse der beiden Studien aufgrund einer erhöhten Anzahl von Spätkomplikationen und dadurch notwendigen Reinterventionen zunächst eher enttäuschend für die endovaskuläre Gruppe ausfielen, haben sich die endovaskuläre Methode und die Prothesen dennoch kontinuierlich weiterentwickelt und zeigen einen klaren Trend zu einer deutlich verbesserten Haltbarkeit mit weniger Reinterventionsmaßnahmen (Blankensteijn et al. 2005).
