Zusammenfassung
Leistenschmerz beim Spitzensport ist ein häufiges und oft schwierig zu behandelndes Problem mit bisweilen langen Ausfallzeiten. Denn die Differenzialdiagnosen sind zahlreich und umfassen unter anderem extraartikuläre (z. B. Muskel- und Sehnenverletzungen) und intraartikuläre Ursachen (z. B. femoroazetabuläres Impingement) sowie die Osteitis pubis. Ebenso sind die pathophysiologischen Einflussfaktoren komplex. Für die effektive Diagnostik ist daher eine enge Zusammenarbeit zwischen Radiologen und Sportorthopäden entscheidend zur Festlegung der bestmöglichen Bildgebung. Meist ist die Magnetresonanztomographie Methode der Wahl. Der Radiologe kann dem Sportorthopäden helfen, eine sog. weiche Leiste korrekt einzuordnen und andere Ursachen wie eine Osteitis pubis als Korrelat herausarbeiten oder ausschließen. Dies Kapitel behandelt die Epidemiologie des Leistenschmerzes beim Spitzensportler und beschreibt die radiologische Diagnostik bei häufigen Differenzialdiagnosen wie Muskel- und apophysären Verletzungen, Stressfrakturen, Osteitis pubis und Labrumpathologien des Hüftgelenkes.
