Zusammenfassung
In einem psychodynamischen Krankheitsverständnis sind pathologisches Erleben und Verhalten durch eine unbewusste Konfliktdynamik gesteuert, die aus signifikanten Belastungen in der Biografie des Subjektes resultieren. Der psychodynamische Zugang ist somit biografisch ausgerichtet und fokussiert auf die aktuellen Auswirkungen der konflikthaften und strukturell vulnerablen Persönlichkeitsbedingungen. Das gilt auch für somatoforme Schmerzsyndrome, die durch anhaltende Körperbeschwerden charakterisiert sind und für die sich keine ausreichende organische Erklärung im Sinne struktureller Organpathologie finden lässt. Psychodynamische Theorien zu somatoformen Störungen, in vormaliger Terminologie funktionellen Störungen, haben unter der Überschrift »Krankheit als Konflikt« (Alexander Mitscherlich) eine lange Tradition, welche die Entwicklung der psychoanalytischen Theoriebildungen des letzten Jahrhunderts widerspiegelt.
