Zusammenfassung
Psychosomatische Medizin stellt eine ärztliche Perspektive dar, die systematisch biologische, psychologische und soziale Einflussfaktoren auf die Entstehung, die Auslösung und den Verlauf von körperlichen Erkrankungen und funktionellen Körpersyndromen untersucht und behandelt. Vielfältige historische Entwicklungstrends, wissenschafts-und versorgungspolitische Einflüsse machen verständlich, dass eine konsensuelle Definition über Wesen und Umfang der psychosomatischen Medizin im Korpus der medizinischen Fächer und in den Versorgungsstrukturen schwer fällt. Zentrale Aufgabenfelder der psychosomatischen Medizin aber bestehen in der Diagnostik und Therapie von auf den diversen Stufen des medizinischen Versorgungssystems häufig komorbiden somatischen Krankheiten und psychischen Störungen, von Somatisierungssyndromen, in einer speziellen therapeutischen Begleitung bei bedrohter Krankheitsverarbeitung und schwierigem Krankheitsverhalten sowie in einer professionellen Unterstützung bei signifikanten psychologischen und psychosozialen Belastungen infolge langwieriger Krankheitsprozesse. Das therapeutische Selbstverständnis der psychosomatischen Medizin betont einen patientenzentrierten Ansatz. In einem biopsychosozialen Krankheitsverständnis sind ihre therapeutischen Verfahren multimodal angelegt. Hinsichtlich der Versorgungsstrukturen im Gesundheitssystem besteht Konsens über eine zu fördernde Verankerung einer allgemeinen psychosomatischen Perspektive in der medizinischen Ausbildung und in den medizinischen Fächern einschließlich einer fachspezifischen Psychosomatik sowie über eine Stärkung von Kompetenzen in der ambulanten psychosomatischen Grundversorgung durch Allgemeinärzte und Fachärzte. Als Spezialfach weist die „Psychosomatische Medizin“ auf den zentralen Wirkbereich eines Konsultation-Liaison-Dienstes. Für die zahlreichen Vermittlungs-, Lehr- und Forschungsaufgaben ist auf universitärer Ebene eine eigenständige Klinik gut zu begründen. Die psychosomatische Medizin versieht entscheidende Versorgungsaufgaben in der psychosomatischen Rehabilitation. Hinsichtlich weiterer stationärer Therapieeinrichtungen für die psychosomatische Akutversorgung besteht in den deutschsprachigen Ländern, insbesondere in Deutschland eine kontroverse Diskussion zwischen den Fächern Psychosomatische Medizin und Psychotherapie einerseits und Psychiatrie und Psychotherapie andererseits.
