Viszerale und HNO-Verletzungen, seltene und schlecht klassifizierbare Formen der Kindesmisshandlung

Zusammenfassung

Während bei misshandelten Kindern relativ häufig Verletzungen der Hals-, Nasen- und Ohrenregion sowie der Mundhöhle und der Zähne auftreten, sind thorakale und (intra-)abdominelle Verletzungen zwar relativ selten, dafür aber bezüglich der Schwere und Lebensgefährlichkeit der Verletzungen von umso größerer Bedeutung. Der Kopf, das Gesicht und insbesondere der HNO-Bereich sowie der Nacken sind bei bis zu 65–75% der misshandelten Kinder betroffen. Etwa die Hälfte betrifft dabei das Gesicht oder die Mundhöhle. Viszerale Verletzungen betreffen ein weites Spektrum an Organen. Bei weitem am häufigsten sind jedoch die des Bauchraums. Sie betreffen etwa 0,5–4% aller Misshandlungsfälle. Nach den ZNS-Verletzungen stellen sie jedoch die zweithäufigste Todesursache bei Misshandlungen dar. Neben dem Spektrum häufiger Formen der Kindesmisshandlung durch stumpfe Gewalt, Schütteltrauma sowie thermische misshandlungsbedingte Verletzungen, ist im Einzelfall an speziell definierte, seltene oder auch ungewöhnliche Formen der Kindesmisshandlung zu denken, zu denen auch das Münchhausen-Syndrom-by-Proxy, absichtliche Intoxikationen und die Genitalverstümmelung zählen
