Zusammenfassung
Grundsätzlich bestehen drei verschiedene Infektionswege: die Kolonisation der oberen Atemwege und nachfolgende oropharyngeale Aspiration respiratorischen Sekrets, die exogene Inhalation von Pathogenen sowie (selten) die hämatogene Streuung. Die Etablierung einer Infektion setzt drei Faktoren voraus: eine eine ausreichend hohe Keimzahl eines pathogenen Erregers, eine Überwindung der angeborenen Immunität der oberen und unteren Atemwege sowie der erworbenen Immunität des Wirts.
Die Mechanismen der Inflammationsreaktion sind komplex und noch nicht vollständig verstanden. Als Folge kann es zu zwei wesentlichen Komplikationen kommen: der akuten respiratorischen Insuffizienz durch Ventilations-/Perfusionsverteilungsstörungen bis hin zur Totraumventilation bzw. einem Shunt sowie zur schweren Sepsis bzw. zum septischen Schock mit der Folge eines Multiorganversagens. Zudem können Pneumonie-assoziierte Komplikationen hinzutreten.
Die offenbar wichtigste Inflammations-assoziierte Komplikation ist kardiovaskulärer Art und äußert sich in Herzinsuffizienz, Myokardischämie bzw. -infarkt und Rhythmusstörungen.
