Zusammenfassung
Die ärztliche Verordnung von Schmerzmitteln hat in den letzten 20 Jahren eine bemerkenswerte Trendwende vollzogen. Nichtopioide Analgetika wurden 30% weniger verordnet, Opioidanalgetika dagegen 3-mal so viel. Das hat dazu geführt, dass jetzt fast doppelt so viele Opioidanalgetika wie nichtopioide Analgetika verschrieben werden. Nicht verschreibungspflichtige, nichtopioide Analgetika wie Acetylsalicylsäure und Paracetamol können nur in Sonderfällen zu Lasten der GKV verschrieben werden. Weit über die Hälfte der Opioidverordnungen entfällt auf die beiden schwachwirksamen Opioide Tramadol und Tilidin/Naloxon. Die Verschreibungshäufigkeit der Tilidin/Naloxon Kombination erreicht fast die aller stark wirkenden Opioidanalgetika. Führende Mittel der starkwirksamen Opioide sind Fentanylpflaster und Oxycodon sowie Hydromorphon, während das als Goldstandard empfohlene Morphin seit Jahren rückläufig ist. Einige Opioide (Methadon, Levomethadon, Buprenorphin) werden in der Substitutionsbehandlung opioidabhängiger Personen eingesetzt.
Bei den nichtopioiden Analgetika ist ein auffälliger Wandel eingetreten. Die Verordnungen von Acetylsalicylsäure und Paracetamol sind in den letzten 10 Jahren um über 60% zurückgegangen, während das rezeptpflichtige Metamizol trotz des weiterhin bestehenden Agranulozytoserisikos fast dreifach häufiger verordnet wurde.
