Zusammenfassung
Eine Pankreasresektion ist die einzige Behandlungsmethode mit kurativem Ansatz bei Patienten mit Pankreas-, periampullären und distalen Gallengangkarzinomen. Auch bei benignen (z. B. endokrinen) Tumoren und der chronischen Pankreatitis nimmt die chirurgische Therapie einen wichtigen Stellenwert ein. Während sich die Letalität der Pankreasresektionen in den letzten Jahrzehnten deutlich reduziert hat und aktuell unter 5% liegt, bleibt die Morbidität weiterhin hoch. Zu den Hauptkomplikationen der Pankreaschirurgie zählen die Pankreasfistel, die arterielle Blutung, die verzögerte Magenentleerung (DGE) sowie Anastomoseninsuffizienzen. Lebensbedrohliche Komplikationen treten v. a nach Pankreaskopfresektionen auf. In diesem Kapitel soll das differenzierte Management der wichtigsten Komplikationen nach Pankreasresektionen aufgezeigt werden.
