Zusammenfassung
Die Hüftkopfnekrose ist die schwerwiegendste Komplikation, die während der Behandlung der angeborenen Hüftluxation auftreten kann. Alle anderen Komplikationen lassen sich auf operativem Wege beheben. Bei einer verspäteten Diagnose kann notfalls eine offene Einstellung der Luxation erfolgen, auch Reluxationen können erneut reponiert werden. Verbleibende Dysplasien und Fehlstellungen des Schenkelhalses sind ebenfalls operativ zu bessern. Tritt aber einmal eine schwerere Nekrose des Kopfes oder der Wachstumszonen ein, so bleibt das Wachstum des Kopfes zurück, er wird flach, breit und entrundet. Auch die Pfanne wird dann auf die Dauer flacher und oft auch steiler durch Behinderung des Wachstums des Pfannenerkers. Die Folge ist eine frühzeitige Arthrose mit allen ihren Beschwerden. Nekrosen können an der Wachstumszone des Schenkelhalses durchgehend oder nur seitlich auftreten. Dann bleibt das Wachstum des Schenkelhalses zurück oder gerät in eine Fehlrichtung. Es kann damit zu erheblichen Valgusstellungen (Abb. 16.1 b) und „Kopf-im-Nacken-Lage“ (Jansen 1925) kommen („subcapital coxa valga“ nach Jones 1977 a) oder zur Coxa vara (Abb. 16.1 d), je nachdem ob die Schädigung mehr die mediale oder die laterale Wachstumszone betraf. Der Trochanter wächst weiter und steht dann zu hoch (Ogden 1974, 1982; Kalamchi u. MacEwen 1980; Siffert 1981; Hirohashi et al. 1982).
