Zusammenfassung
Unter den vielen Operationen zur Behandlung des Hallux valgus haben die Osteotomien ihren eigenen Stellenwert. In erster Linie kann damit das Metatarsophalangealgelenk erhalten bleiben. Voraussetzung dafür ist allerdings, daß dessen Zustand eine Erhaltung überhaupt sinnvoll erscheinen läßt. Metatarsalosteotomien können distal oder proximal oder als Doppelosteotomie geplant werden. Eine Schaftverlängerung oder -Verkürzung ist möglich, gleichzeitig auch die Beeinflussung des Intermetatarsalewinkels. Dies alles setzt eine exakte Planung voraus. Wie bei anderen Osteotomien stellt sich immer wieder das Problem der Fixation, damit die vollständige Konsolidierung raschmöglichst erfolgt. Der Operationsaufwand soll in angemessenem Aufwand stehen zum erwarteten Endergebnis.
