Zusammenfassung
Pankreassteine werden, in Abhängigkeit von der Krankheitsdauer, bei über 40% der Patienten mit chronischer Pankreatitis beobachtet [3]. Der steinbedingten Obstruktion wird eine pathogenetische Rolle für die Schmerzsymptomatik zugeschrieben. Die bisherigen Erfahrungen mit endoskopisch implantierten Drainagen bei der obstruktiven Pankreatitis bestätigen die chirurgisch-operative Erfahrung, daß durch die Beseitigung der Obstruktion im Pankreasgang Schmerzen sofort beseitigt werden und Entzündungsschübe verhindert werden können. Endoskopisch-operative Manöver versagen jedoch bei inkrustierten Steinen oder Stenosen. Insbesondere bei diesen Problemfällen ist die extrakorporale Stoßwel- lenhthotripsie (ESWL) ein neues vielversprechendes Therapieverfahren [1, 4, 5]. Da klinisch bedeutsame duktale Pankreaskonkremente in aller Regel gut darstellbar und mit einem Ultraschall-gesteuerten Stoßwellenlithotripter gut zu orten sind, haben wir bei bisher 9 Patienten die Möghchkeit der sonographisch geführten Pankreasstein-ESWL überprüft.
