Zusammenfassung
Der Beitrag beschreibt Ziele, Vorgehensweise und Ergebnisse eines mehrjährigen Modellversuchs zur wohnortnahen und komprehensiven Versorgung von Rheumakranken (chronische Polyarthritis) in der Bundesrepublik Deutschland. In 5 Regionen wurde die Versorgungsinfrastruktur fortentwickelt und das Dienstleistungsangebot — v. a. im „komplementären“ Bereich — verbessert. Die wissenschaftliche Begleitforschung beinhaltete sowohl eine Implementationsanalyse als auch eine Prozeß- und Effektevaluation.
Die Studie belegt zunächst die „Machbarkeit“ des Versorgungsausbaus und die weitgehende „Akzeptanz“ bei allen Beteiligten; weniger dagegen die intensive und kontinuierliche Kooperation (z. B. der niedergelassenen Ärzte). Die patientenbezogene Effektevaluation (n = 554), die sich auf 3 Erhebungswellen stützt, hat eine Stabilisierung (mit leichten Verbesserungstendenzen) von somatischen, psychischen und sozialen Indikatoren im Gesamtkollektiv ergeben. Dies entspricht den Erwartungen im Rahmen von „Interventionen im offenen Versorgungsfeld“.
Eine kritische Rückschau auf die Durchführung der Studie läßt v. a. die Tatsache sichtbar werden, daß in inhaltlich-konzeptueller wie in methodischer Hinsicht vielfach eine zu enge Orientierung an dem „Modell“ klinischer Studien erfolgt ist. Die Entwicklung spezifischer eigener Standards für Studien der hier beschriebenen Art ist für die Zukunft dringend zu empfehlen.
Summary
The following contribution describes the aims, procedures, and results of a case of „experimental“ improvements of local ambulatory service provisions for rheumatic patients (chronic polyarthritis) in West Germany. During a period of 6 years the respective local infrastructure and the (ambulatory) service delivery were improved in five different sites throughout the country. Scientific research accompanied all developments. It included an implementation analysis as well as a formative and impact evaluation.
The study confirms first of all the feasibility and the acceptance of the change — less so the continuous cooperation of all relevant actors (e. g., of the GPs). The patient impact analysis (n = 554) was done as a three-phase study. It showed a stabilization of health status and psychic and social wellbeing over time — with slight trends to improvement — in the total sample. This result corresponds to the expectations about effects in the context of such a „field intervention study“.
A critical review of the study reveals in some respects a too close orientation to the „clinical study model“, which is often not an adequate design for a field study such as this. Thus, it is recommended to develop more specific substantial concepts and methodological designs for this type of intervention and service research.
