Zusammenfassung
In den Industrienationen gehören die Herzinsuffizienz und der Herzinfarkt nach wie vor zu den primären Todesursachen mit der größten Prävalenz. Nach einem Infarkt, der durch eine Durchblutungsstörung des Herzmuskels (Myokard) ausgelöst wird, steht die schnelle Wiederherstellung der Perfusion des betroenen Gewebes im Mittelpunkt der Therapie. Im Folgenden werden an Hand eines einfachen Modells der Hämodynamik des Herzkreislaufsystems, das sich auf die für das Verständnis wesentlichen Elemente konzentriert, klinisch relevante Parameter zur Beurteilung der Herzfunktion abgeleitet und der Wirkmechanismus heutiger mechanischer Herzunterstützungssysteme erarbeitet. Mit der abschließenden Darstellung der hämodynamischen Wirkung wird der klinische Nutzen der mechanischen Herzunterstützung an Hand der Impella®-Pumpentechnologie im pV-Diagramm veranschaulicht.
