Zusammenfassung
Bei der vorherrschenden asymmetrischen Kriegsführung insbesondere in den aktuellen NATO-Einsätzen wie auch bei terroristischen Anschlägen gegen die Zivilbevölkerung sind Explosionsverletzungen und die dadurch resultierenden thermomechanischen Kombinationsverletzungen eine häufige Verletzungsursache in militärischen Konflikten und in Krisenregionen. Es hat sich gezeigt, dass durch die konsequente Umsetzung präklinischer Konzepte (PHTLS, TCCC), zunehmend kürzeren Transportzeiten und der Anwendung der »damage control surgery« (DCS) innerklinisch, ein Absinken der Mortalitätsrate erreicht werden kann, so dass auch komplexeste Verletzungsmuster überlebt werden. Unabdingbar im Rahmen der Behandlung sind genaue Kenntnisse der Pathophysiologie und insbesondere der Besonderheiten dieser Verletzungsentität.
