Zusammenfassung
Alle zehn Jahre hat sich auf dem Gebiet der Inselzelltransplantation bei Typ-1-Diabetes ein Innovationsschub ereignet. Vor 1990 wurde das Maximalziel Insulinunabhängigkeit nach Transplantation in weniger als 10 % der Fälle erreicht, Ende 1999 bei etwa jedem vierten Patienten. Mit Einführung des sog. Edmonton-Protokolls um die Jahrtausendwende begann eine Erfolg versprechende neue Ära. Mehr als 10 Jahre danach ist die Inselzelltransplantation für Typ-1-Diabetiker an der Dialyse, nach bereits erfolgter Nierentransplantation oder bei Patienten mit schwer einstellbarem Diabetes und Hypoglykämie-Syndrom heute eine geeignete und oft letzte Therapiemöglichkeit Eine weitere Indikation ist die autologe Inselzelltransplantation bei Patienten mit chronischer Pankreatitis, bei denen eine Pankreasresektion geplant ist.
