Zusammenfassung
Laser und lichtemittierende Geräte spielen inzwischen eine entscheidende Rolle bei der Behandlung von Vitiligo und anderen Hypopigmentierungen. Depigmentierende gütegeschaltete Laser können für die Behandlung von verbliebenen pigmentierten Flächen bei der generalisierten Vitiligo eingesetzt werden. Ablative Laser sind ein Instrument zur Vorbereitung des Transplantatbetts bei den segmentalen und lokalisierten Formen der Vitiligo oder bei genetischen hypomelanotischen Störungen wie dem Piebaldismus. Der 308-nm-Excimerlaser und die 308-nm-Excimerlampe sind die beiden Geräte, die den größten Fortschritt für die Behandlung von hypomelanotischen Hautveränderungen gebracht haben. Mit ihren im Vergleich zur herkömmlichen Phototherapie höheren Repigmentierungsraten und der gezielten Behandlung dieser Läsionen unter Aussparung der Hautumgebung liegt ihre wichtigste Einschränkung bei der zu behandelnden Gesamtfläche. In naher Zukunft wird man sicherlich neue Wellenlängen für die Steigerung der Repigmentierung einsetzen. Der 632,8-nm-Heliumneonlaser hat bereits vielversprechende Resultate erbracht, die es weiter zu untersuchen gilt.
