Zusammenfassung
Die Indikationen zur offenen operativen Behandlung der Nierenarterien (NA) sind heute deutlich eingeschränkt, da die endovaskulären Verfahren (Dilatation und Stenting) als Therapie der ersten Wahl gelten. Indikationen finden sich heute noch bei rezidivierenden In-Stent-Stenosen, bei Aneurysmen, spontanen Dissektionen der NA, Typ-B-Aortendissektionen und als additive Maßnahme bei ohnehin erforderlicher Freilegung der Aorta. Die Rekonstruktionsmethoden sind vielfältig: Sie umfassen die Embolektomie, die transaortale und offene Desobliteration sowie diverse Bypassverfahren, einschließlich der Ex-situ-Rekonstruktion.
Ebenso ist im Rahmen des viszeralen Debranchings durch Aortenstentprothesen eine Bypassführung von der infrarenalen Aorta oder den Beckenarterien zu den Nieren- und Vizeralarterien möglich. Gefürchtetste Komplikation bei Eingriffen an den NA ist das akute Nierenversagen, das mit einer hohen Sterblichkeitsrate verbunden ist. Protektiv auf die Ischämietoleranz der Niere wirkt die Spülperfusion mit 4 °C kalter Ringerlösung unter Zusatz von PGE1.
