Zusammenfassung
Für die EEG-Veränderungen sind in erster Linie die Lokalisation und Ausbreitung der entzündlichen Erkrankung von Bedeutung. Maßgebend ist, in welchem Umfang die weiße und die graue Substanz des Hirns betroffen werden (Gloor et al. 1968; ◘ Abb. 11.1). Bei einem Befall der grauen Substanz ist der EEG-Befund davon abhängig, ob vorherrschend die Hirnrinde oder in besonderem Maße subkortikale Kerngebiete erkranken. Bakterielle Entzündungen führen auch in schweren Fällen meistens nur zu uncharakteristischen Allgemeinveränderungen, teilweise auch zu epileptischen Erregungssteigerungen. Bei Meningitiden und Enzephalitiden ist die Entwicklung von Herdbefunden in der Regel ein Hinweis auf eine vaskulitische Komplikation (► Kap. 14). Dagegen können virusbedingte Enzephalitisformen besondere EEG-Syndrome verursachen, die sich dann oft auch neuropathologisch auf ein besonderes Schädigungsmuster der neurotropen Virusinfektion oder der parainfektiösen immunologischen Reaktionen zurückführen lassen. Einige dieser EEG-Befunde können diagnostisch richtungweisend sein.
