Zusammenfassung

Das Kompartmentsyndrom ist eine zu Recht gefürchtete klinische Entität, die sich im Gefolge einer Gewebetraumatisierung unterschiedlichster Ätiologie entwickeln kann. Es handelt sich hierbei um einen Symptomenkomplex, der aus einer Erhöhung des Gewebebinnendruckes in einem anatomisch funktionell abgeschlossenen Raum (Kompartment) resultiert und somit eine Störung der Mikrozirkulation bewirkt mit der Folge einer neuromuskulären Funktionsstörung. Unbehandelt führt das Kompartmentsyndrom zu einer irreversiblen Ischämie mit Muskelnekrose und Nervenschädigung. Diese endet letztlich in einer ischämischen Muskelkontraktur mit deletären Folgen für die Funktion der Extremität. Dieses Vollbild der ischämischen Kontraktur wurde von Richard von Volkmann in seiner klassischen Arbeit 1881 beschrieben und ist deshalb für den Kliniker eng mit dessen Namen verbunden.
