Zusammenfassung
Knochenverluste können sowohl mit auto- oder allogenem Knochen aufgefüllt werden, wie auch mit einem nicht vaskularisierten, freien, kortikospongiösen Span behandelt werden. Langstreckige Substanzverluste können mit mikrovaskulär angeschlossenen, autogenen oder allogenen, vaskularisierten Transplantaten überbrückt werden. Segmentale Unterbrechungen der knöchernen Kontinuität an langen Röhrenknochen können mit Hilfe der Distraktionsosteogenese behandelt werden [1]. Die besten biologischen Voraussetzungen zur Defektauffüllung bietet die autologe Spongiosatransplantation. Sie ermöglicht zudem den grössten Heilungserfolg [2]. Aufgrund der limitierten Verfügbarkeit besteht jedoch auch eine Nachfrage nach Alternativen. Die Verwendung von sogenannten Knochenbankmaterialien als allogener Knochenersatz erwies sich in den letzten Jahren als zunehmend problematisch. Unerwünschte Immunreaktionen, ungewollte Infektionsübertragungen [3, 4] und der nicht unerhebliche Kostenfaktor der Knochenbanken schränkten den Gebrauch deutlich ein [5]. Gerade die Gefahr der Infektionsübertragung führte zu einem erneuten Auftrieb bei der Suche nach Materialien, die natürlichen Knochen adäquat ersetzen können und die gleichzeitig in grossen Mengen verfügbar sind.
