Résumé

Les travaux consacrés aux lésions fraîches de l’appareil fléchisseur ont montré que les résultats des réparations primitives sont globalement meilleures chez l’enfant (3, 4, 7, 9, 10 - 15, 18, 20, 21, 24, 25). Néanmoins, il serait faux de croire que le terrain pédiatrique confère à cette chirurgie une accessibilité technique qu’elle n’aurait pas chez l’adulte. En effet, et surtout chez de jeunes enfants, l’impossibilité de tout contrôle de la mobilisation postopératoire rend encore plus cruciale la qualité de la réparation initiale, surtout en zone II. En revanche, il est vrai que cette chirurgie est moins souvent source de raideur articulaire surajoutée — conséquence de l’immobilisation — que chez l’adulte. De nombreux points tels que le type et la durée de l’immobilisation, la nécessité d’une mobilisation précoce ainsi que l’influence de l’âge et des lésions associées sur le pronostic final restent encore mal connus.
