Substance Abuse in Psychiatric Patients

Etiological, Developmental, and Treatment Considerations

Substance Abuse and Psychopathology

Part of the book series: Applied Clinical Psychology ((ACPH))

Abstract

Alcoholism and drug abuse are known to occur in combination with a number of other disorders. Perusal of the recent Diagnostic and Statistical Manual (American Psychiatric Association, 1980) indicates that substance abuse problems may be among those exhibited in a number of psychiatric disorders, as in many of the personality and anxiety disorders and in the affective disorders. Many of the chapters in this book confirm this fact. Additionally, research by this writer (Alterman, Erdlen, McLellan, & Mann, 1980; Alterman, Erdlen, & Murphy, 1981) and others (McLellan, Druley, & Carson, 1978) have revealed that problems of alcoholism and drug abuse are a frequent complicating condition in schizophrenia. In this world of specialization, the existence of alcoholism or drug abuse as secondary diagnoses for persons with other primary diagnoses, does, indeed, represent a complicating circumstance. As Alterman’s research on a VA psychiatric inpatient population indicated (Alterman, Erdlen, McLellan, & Mann, 1980; Alterman et al, 1981), general psychiatric staff feel, and are generally, ill equipped to treat psychiatric patients with concomitant alcohol or drug abuse problems. Although informal programs certainly exist for the treatment of conjoint serious psychiatric and addiction problems, there are almost no programs designed specifically for this purpose.
