Zusammenfassung
Mit dem Alter steigt das Risiko eines Patienten, eine schwere perioperative Komplikation zu erleiden. Die Wahrscheinlichkeit einer Komplikation wird dabei vor allem durch das Spektrum der Begleiterkrankungen und die körperliche Belastbarkeit des Patienten bestimmt. Diese Tatsachespiegeltsich in einer hohen Inzidenzvon perioperativen Komplikationen bei einem ASA-Score von 3 oder höher wieder. Die Anästhesie stellt, ebenso wie die perioperative Stressreaktion, eine Anforderung an die körperliche Belastbarkeit dar. Die Bestimmung der Belastungsreserve ist daher für die Einschätzung des perioperativen Risikos, aber auch für die Festlegung des anästhesiologischen Managements von großer Bedeutung. Die Belastungsreserve der Organe, die auch als »funktionelle Reserve« bezeichnet wird, ist definiert als die Differenz zwischen der Organfunktion unter Ruhebedingungen und dem maximal erreichbaren Funktionslevel unter Belastung (⊡ Abb. 45.1).
