Objectif
Le bon usage des antibiotiques est essentiel chez la personne âgée. Le but de notre étude était d’évaluer la plus grande fréquence de prescription d’amoxicilline/ acide clavulanique (Ax/Ac), l’antibiotique le plus prescrit dans les hôpitaux français chez les personnes âgées au service d’accueil des urgences (SAU).
Patients et méthodes
Étude rétrospective de janvier à novembre 2012 dans le SAU du CHU Henri Mondor, en région parisienne. Tous les patients qui ont reçu Ax/Ac ont été inclus et répartis en deux groupes (patients jeunes <75 ans et patient âgés [PA] ≥75 ans) qui ont été comparés. Résultats: Sur la période d’étude, 41 638 patients ont été admis au SAU. Parmi eux, 6669 avaient ≥75 ans (16,0 %) et 571 avaient reçu l’Ax/Ac (1,4 %). L’Ax/Ac était cinq fois plus prescrit chez les patients âgés que chez les patients jeunes (OR 5,2, IC 95 % 4,4 à 6,2, p<0,001): 284 prescriptions chez les patients âgés (49,7 % des prescriptions de l’Ax/Ac et 4,3 % des PA) et 287 chez les patients jeunes (50,3 % des prescriptions de l’Ax/Ac et 0,7 % des jeunes adultes). Le sexe féminin, les infections respiratoires, l’hyperleucocytose et les hospitalisations étaient significativement plus fréquentes chez les patients âgés, tandis que l’utilisation en prophylaxie d’Ax/Ac était plus fréquente chez les patients jeunes.
Conclusion
Il existe une fréquence plus élevée de prescriptions d’Ax/Ac chez les patients âgés. Ceci est probablement expliqué en partie par la plus grande fréquence des infections respiratoires et les difficultés de diagnostic de l’infection dans cette population, en particulier au SAU. La 1re ligne antibiotique initiée au SAU doit être rigoureuse chez les PA afin d’éviter un traitement inadapté, un échec du traitement et l’émergence de résistances.
Abstract
Aims
The proper use of antibiotics is essential in the elderly. Prescription is often initiated in the Emergency Department (ED). The aim of our study was to estimate the higher number of prescriptions of antibiotics in the elderly in the ED, taking the example of the amoxicillin/clavulanic acid (Ax/Ac), the most prescribed antibiotic in French hospitals.
Procedure
Retrospective study between January and November 2012 in the ED of Henri Mondor Universitary Hospital, Paris region, France. All patients who received Ax/Ac were included and divided in two groups (young <75 years and elderly ≥75 years), and then compared.
Results
During the study period, 41,638 patients were admitted to the ED. Among them, 6,669 were older than 75 years (16.0 %) and 571 received Ax/Ac (1.4%). Ax/Ac prescription was 5 times higher in the elderly than in the young (OR 5.2, 95% CI 4.4-6.2, p<0.001): 284 prescriptions in the elderly (49.7% of Ax/Ac prescriptions and 4.3% of the elderly patients) and 287 in the young (50.3% of Ax/Ac prescriptions and 0.7% of young adults). Female gender, respiratory infections, leukocytosis and hospitalization were significantly more frequent in elderly, while prophylaxis use was more common in young people.
Conclusion
There is a higher prescription of Ax/Ac in elderly patients. This is likely explained by the higher frequency of respiratory tract infections, multiple comorbidities and the difficulties of diagnosis of infection in this population, especially in the ED. First line antibiotic regimen must be particularly rigorous in the elderly to avoid unnecessary or inappropriate antibiotic therapy, treatment failure and emergence of resistance.
