Avoid common mistakes on your manuscript.
In 2020 is de NDF Voedingsrichtlijn volledig herzien. In deze richtlijn staan voedingsadviezen voor mensen met (een hoog risico op) diabetes. Een persoonsgerichte aanpak staat hierbij centraal. Naast verschillende voedingsstoffen en voedingsmiddelen zijn acht verschillende voedingspatronen in detail uitgewerkt. Zorgverleners kunnen voedingsadviezen hiermee afstemmen op individuele wensen en behoeften. Omdat er de afgelopen jaren veel nieuw onderzoek is verschenen, verschillende voedingshypes in de spreekkamer voorbijkomen en er nieuwe medicatie op de markt is, is de richtlijn van een update voorzien. De update werd uitgevoerd door een multidisciplinaire expertgroep. Er is nieuwe wetenschappelijke literatuur gezocht en deze is gebundeld met ervaringen uit de praktijk. Het literatuuronderzoek heeft niet geleid tot nieuwe aanbevelingen, maar geeft wel nieuwe bewijsvoering en onderbouwing van de aanbevelingen.
In de NDF Voedingsrichtlijn staan voedingspatronen centraal. Vanwege de grote hoeveelheid vragen rondom koolhydraatbeperkte voedingspatronen is dit onderwerp in detail opnieuw bekeken. Wat zijn de nieuwste inzichten over koolhydraatbeperking bij diabetes?
Nieuwe onderzoeken ondersteunen de aanbeveling van de NDF dat er geen bepaalde hoeveelheid koolhydraten aan te bevelen is voor mensen met diabetes. Wel blijven voedingspatronen met een zeer lage koolhydraatinname, zoals ketogene diëten, niet aan te raden. Bij koolhydraatbeperking blijft het van belang om de glucoseverlagende medicatie goed af te stemmen op de hoeveelheid koolhydraten, om zo hypoglykemie te voorkomen.1 Naar koolhydraatarme diëten bij type 1-diabetes wordt beduidend minder onderzoek gedaan dan naar de effecten bij mensen met type 2-diabetes. Op basis van een nieuwe meta-analyse, waarin de effecten worden samengevat van koolhydraatbeperking bij volwassenen met type 1-diabetes, blijft het advies dat minder dan 70 gram koolhydraten voor mensen met type 1-diabetes ongeschikt blijkt vanwege het risico op ketoacidose en hypoglykemie.2
Een ander onderwerp dat uitgebreider aan bod komt in de update zijn de GLP-1-agonisten. Deze medicatie kreeg bij het uitkomen van de richtlijn in 2020 minder aandacht. Wat staat hierover in de update vermeld en waarom worden ze besproken in een voedingsrichtlijn?
Sommige GLP-1-agonisten zorgen voor een verzadigingsgevoel en verminderen de voedselinname. Bij het gebruik van een GLP-1-agonist verandert soms ook de smaakperceptie. Vanuit de praktijk is er vraag naar specifieke voedingsadviezen bij het gebruik van GLP-1-agonisten. Vooral bij aanvang van de behandeling kunnen maag- en darmklachten − zoals misselijkheid, braken en diarree − optreden. In de update van de richtlijn worden nu ook adviezen bij misselijkheid omschreven. Er is nog wel te weinig onderzoek gedaan naar de interactie tussen voedingspatronen en GLP-1-agonisten om specifieke voedingsadviezen te kunnen geven aan mensen die GLP-1-agonisten gebruiken.
Voor veel mensen met type 2-diabetes is gewichtsbeheersing een belangrijk aspect van de behandeling. Zijn er nieuwe conclusies over de effectiefste manier om gewicht te verliezen bij type 2-diabetes?
De umbrella-review van Churuangsuk (2022) heeft verschillende meta-analyses samengevat, waarin verschillende voedingsinterventies met elkaar werden vergeleken. Hieruit komt naar voren dat interventies met very low calorie diets of maaltijdvervangers voor het meeste gewichtsverlies zorgen. Bij interventies waarbij maaltijdvervangers werden gebruikt als start van een interventie is de kans op langdurige verbetering van glucosewaarden groot.3 Wel is het belangrijk om mensen adequaat te begeleiden in het gebruik van maaltijdvervangers, en ook in het traject erna.
Een andere methode die door sommigen wordt gebruikt om gewicht te verliezen, is intermitterend vasten. Zorgt intermitterend vasten voor meer gewichtsverlies en betere glucosewaarden dan andere diëten?
In de update worden twee nieuwe meta-analyses van gerandomiseerde interventieonderzoeken meegenomen die de recentste studies naar intermitterend vasten samenvatten. Hoewel intermitterend vasten op korte termijn voor iets meer gewichtsverlies lijkt te zorgen dan continue energierestrictie, werd er geen verschil in HbA1c of nuchtere bloedglucose gevonden.4,5
Een ander onderwerp waarnaar veel onderzoek wordt gedaan, is ultrabewerkte voeding. Wat betekenen deze producten voor het risico op en de behandeling van diabetes?
Ultrabewerkte voeding of ultra-processed foods is een voedingsgroep waaronder veel ongezonde producten vallen, zoals frisfranken, fastfood, snoep en snacks. Er zijn geen trials waarin het effect van ultra-processed foods in een gecontroleerde setting wordt onderzocht bij mensen met diabetes. Observationeel bewijs geeft verbanden aan tussen een hoge inname van zulke producten en ziekten zoals coronaire hartziekten, type 2-diabetes en verschillende vormen van kanker. Hoewel dit zorgwekkend is, concludeert de werkgroep dat we op dit moment nog te weinig over ultra-processed foods weten om een specifieke aanbeveling te kunnen doen. In lijn met adviezen van de European Association for the Study of Diabetes blijft het raadzaam om vooral minimaal bewerkte voedingsmiddelen − zoals volle granen, groenten, heel fruit, peulvruchten, noten, zaden en niet-gehydrogeneerde plantaardige oliën − te bevorderen, terwijl de consumptie van bewerkt (en rood) vlees, suikerhoudende dranken, snoep, bewerkte snacks en geraffineerde granen tot een minimum wordt beperkt.1
Het hoofdstuk zoetstoffen is ook bijgewerkt in de update. Kunnen mensen met diabetes beter producten met zoetstoffen gebruiken dan producten met suiker?
Zoetstoffen zitten vooral veel in (light) frisdranken. Voor dranken geldt: ongezoet water, thee of koffie zijn de beste dorstlessers. Omdat zoetstoffen minder energie bevatten dan suiker, bevatten producten waar de suiker is vervangen door zoetstof vaak minder calorieën. Nieuw onderzoek bevestigt dat het drinken van dranken met zoetstoffen in plaats van suikerhoudende dranken samenhangt met een lager lichaamsgewicht en een gunstiger cardiometabool risicoprofiel.6,7 Het vervangen van dranken met zoetstof door water zorgt echter voor nog een lager HbA1c.6 Uit een nieuwe systematische review en meta-analyse van de WHO (283 studies) blijkt daarnaast dat het vervangen van suiker door zoetstof op de lange termijn niet helpt om gewicht te verliezen of een gezond gewicht te behouden.8 Internationale nieuwe richtlijnen bevelen dan ook alleen het gebruik van caloriearme zoetstoffen aan wanneer iemand het gebruik van suiker zou willen verminderen.
En hoe zit het dan met fructose? Hangt een hoge inname niet samen met leververvetting?
Op de lange termijn zijn er inderdaad aanwijzingen dat leververvetting vaker voorkomt bij mensen die chronisch veel fructose uit fris- of vruchtendrank binnenkrijgen.9 Vooral bij mensen met type 2-diabetes lijkt dit verband sterk te zijn. Daarnaast zien we ook dat mensen die veel fructose binnenkrijgen een hoger risico op type 2-diabetes hebben.10 Het gaat hierbij om extreme hoeveelheden fructose van ongeveer 70 gram per dag (14% van energie-inname).
Voor het eten van stukken fruit, waarin ook fructose zit, worden vooral beschermende effecten gevonden; het risico op type 2-diabetes gaat omlaag met 10% bij een inname van fruit tot 200-300 g/dag. Boven deze hoeveelheid is er geen voordeel meer voor het verhogen van de inname.11 Er is geen onderzoek waarin werd onderzocht wat de maximale hoeveelheid fruit zou zijn voor mensen met diabetes. De aanbeveling blijft dan ook om twee stuks fruit per dag te consumeren.
