In the October 2005 publication of the above-referenced article, volume 29, issue 10, the authors have identified an error in their calculations. In two places on page 1243 they provide an example of a procedure with a mortality rate of 0.7%. In truth, the correct mortality rate for this example is 6%, when calculated correctly. This error in calculation does not affect the applicability of the example case or the conclusions of the article.
