Zusammenfassung
Hintergrund
Durch die Somatostatinrezeptorszintigraphie ist es möglich, neuroendokrine Tumoren zu lokalisieren, welche durch die konventionellen bildgebenden Verfahren nicht entdeckt werden können.
Patienten und Methoden
Bei 270 Patienten mit der Verdachtsdiagnose eines gastroenteropankreatischen Tumors (GEP-Tumor) wurden 400 Rezeptorszintigraphien durchgeführt. 70 Patienten (38 Frauen, 32 Männer; Alter 28 bis 74 Jahre [56±12,6]) wurden operiert und der Verlauf über mindestens zwei Jahre dokumentiert. Das Ziel der Untersuchungen war sowohl der Vergleich der präoperativen Somatostatinrezeptorszintigraphie mit den radiologischen Verfahren als auch mit der intraoperativen Metastasenlokalisation mittels handgeführter Gamma-Sonde.
Ergebnisse
Der szintigraphische Nachweis von Primärtumoren gelang bei allen Patienten. Leber- und Lymphknotenmetastasen wurden mit einer Sensitivität von 94% bzw. 95% rezeptorszintigraphisch lokalisiert. Bei sieben Patienten wurde eine intraoperative Sondenlokalisation durchgeführt: Mit diesem Verfahren konnten insgesamt 35 Herde detektiert werden. Durch Palpation wurden 15, mit Hilfe der radiologischen Verfahren (Sonographie, CT) elf und mit der präoperativen Rezeptorszintigraphie wurden 27 Herde nachgewiesen.
Schlußfolgerung
Die Somatostatinrezeptorszintigraphie ist den radiologischen Verfahren in der Lokalisation von kleinen, aber metabolisch endokrin wirksamen GEP-Tumoren überlegen und sollte daher bei begründetem Verdacht der CT vorangestellt werden. Möglicherweise kann die intraoperative Metastasenlokalisation häufiger eine komplette und minimalinvasive chirurgische Revision und Resektion ermöglichen.

[111In-DTPA-D-Phe1]-pentetreotide scintigraphy is able to detect neuroendocrine tumors not shown by radiological methods.
Patients and Methods
In 270 patients with neuroendocrine gastroenteropancreatic tumors (GEP tumors) 400 somatostatin rezeptor scintigraphies were performed. 70 patients (38 female, 32 male; aged 28 to 74 [56±12,6] years) underwent surgery and follow-up over 2 years. The aim of the present study was the comparison of preoperative somatostatin receptor scintigraphy with radiological methods (sonography, CT) and intraoperative localization of GEP tumors with a hand-held gamma-probe.
Results
Somatostatin receptor scintigraphy was successful in localizing primary tumors in all patients. Liver- and lymph node metastases could be visualized with a sensivitity of 94 and 95 percent. In 7 patients 35 lesions could be identified by intraoperative tumor localization using a hand held gamma probe. Radiological methods identified only 11, surgical palpation 15 and preoperative somatostatin receptor scintigraphy 27 lesions.
Conclusion
Somatostatin receptor scintigraphy improves detection of small and occult GEP tumors. Intraoperative probe counting with a hand-held gamma probe can identify tumors even when they are small and impalpable, but receptor positive.
