Ce n’est que depuis peu de temps que le traitement endoscopique des lésions kystiques péripancréatiques par drainage transgrastromural n’est plus limité aux lésions à contenu principalement liquide. Nous décrivons la méthode de nécrosectomie endoscopique r rétropéritonéale avec accès transgastrique direct par débridement des nécroses solides péripancréatiques infectées. Les lésions kystiques rétropéritonéales sont d’abord ponctionnées sous ultrasonographie endoscopique. Ensuite, une large ouverture du canal transmural est pratiquée à l’aide d’un ballon de dilatation permettant l’accès d’un endoscope thérapeutique. Enfin, le tissu solide nécrosé est mobilisé puis retiré.
Sur les 14 patients traités, une seule complication sévère a été relevée chex un patient qui présentait une hémorragie sévère de l’artère splénique 12 heures après le débridement. Après réparation chirurgicale, l’évolution s’est déroulée favorablement sans autre complication. Aucun cas de mortalité n’est à déplorer. Comparée à l’acte chirurgical, la nécrosectomie endoscopique rétropérinéale est une alternative thérapeutique peu in vasive pour certains patients souffrant de nécrose péripancréatique infectée.
Summary
Endoscopic treatment of peripancreatic cystic lesions by transgastromural drainage until recently has been restricted to lesions with mainly liquid contents. We describe endoscopic retroperitoneal necrosectomy using the direct transgastral access for debridement of solid infected peripancreatic necrosis. Retroperitoneal cystic lesions were first punctured under guidance of endoscopic ultrasound. A wide fenestration allowing access for a therapeutic gastroscope to the retroperitoneal cavity was created by balloon dilatation of the transmural channel. Solid necrotic tissue was mobilized and removed.
In 14 patients treated, the only severe complication was acute bleeding from the splenic artery in one patient 12 hours after endoscopic debridement. Surgical repair resulted in an otherwise favourable outcome without further complications. There was no procedure related mortality. Endoscopic retroperitoneal necrosectomy may be a minimally invasive therapeutic alternative to surgery in certain patients with infected peripancreatic necrosis.
