Conclusion
Vingt ans après son apparition, l’EE est à un tournant de son évolution. Essentiellement descriptive à ses débuts, elle est devenue diagnostique et thérapeutique. L’essor de nouvelles techniques moins invasives, mais aussi peut-être moins précises, doit nous inciter à démontrer son intérêt en terme d’impact bénéfique sur la prise en charge des malades, aussi bien sur le plan diagnostic, thérapeutique qu’économique. La généralisation de la ponction permettra d’accroître la précision diagnostique. L’apport de la minisonde dans les pathologies ampullaires et les TIPMP guide au mieux la chirurgie. La différenciation entre les masses tumorales et inflammatoires sera mieux appréciée à l’aide du doppler et des produits de contraste.
C’est en sachant évoluer, progresser que l’échoendoscopie gardera une place privilégiée dans la stratégie diagnostique et thérapeutique de la pathologie bilio-pancréatique.
Conclusion
Twenty years after its introduction, endoscopic ultrasonograpy has reached a turning point. Basically a descriptive method during its early phase of development, EUS has now become a diagnostic and therapeutic tool. The advent of new less invasive techniques that may also be less precise, points to the importance of demonstrating the beneficial effect of EUS on patient management, in terms of diagnostic, therapeutic and economic impact. The widespread use of fine-needle aspiration will help improve diagnostic precision. Miniprobes will also contribute to guiding surgery in selected cases. Differentiation between a tumor and an inflammatory mass can be best assessed with the Doppler tool and use of contrast agents. By continuing to change and progress, endoscopic ultrasonography will maintain is prominent role in the diagnostic and therapeutic strategy for bilopancreatic disease.
