Les applications principales de l’échographie colique sont le diagnositic et le suivi de la maladie de Crohn, de la colite ulcérative, de la diverticulite et de l’iléus, ainsi que des formes transmurales de l’entérocolite infectieuse. Sa valuer dans la recherche de tumeurs coliques est arbitraire, et elle est négligeable dans d’autres maladies.
L’analyse de la transformation de la paroi intestinale suit une typologie orientée selon les modifications pathologiques des couches anatomiques. Sept types de ces modifications ont été décrites et mis en relation: 1) type accentué, 2) type gyriforme, 3) type hypo-échogène avec contours flous ou 4) avec contours nets, 5) type à dominante sub-mucosale, 6) type à dominantemuscularis et 7) destruction de couche.
L’échographie montrera les manifestations extra-intestinales de la maladie de Crohn et de la diverticulite (rayures mésentériques hypoéchogènes et infiltration séreuse/omentale hyper-échogène, fistules et abcès). Cette technique peut estimer le degré de sténose et son impact sur le transit depuis un ralentissement insignifiant jusqu’á un sub-iléus. En diagnostic différentiel, elle montrera la transformation continue faiblement échogène de la paroi dans les accès sévères de colite ulcérative, son extension ascendante, ses phases de rémission et elle m mettra également en évidence l’épaississement gyriforme ou accentué de la paroi et l’augmentation de taille des ganglions lymphatiques de proximité dans l’entérocolite infectieuse.
Summary
The mainstay of colonic sonography is the diagnosis and follow-up of Crohn’s disease, ulcerous colitis, diverticulitis and ileus, as well as transmural forms of infectious enterocolitis. Its value in the search for colonic neoplasms in arbitrary, in other diseases neglectable.
The analysis of bowel wall transformation follows a typology oriented according to the pathological changes in the anatomic layers. Seven such typical forms are described and put into relation: 1) accentuated type, 2) gyriform type, 3) hypoechogenic type with faded borders or 4) with sharp borders, 5) submucosa-dominant type, 6) muscularis-dominant type and 7) destructed layering.
Ultrasound will show the extra-intestinal manifestations of Crohn’s disease and diverticulitis (hypo-echogenic mesenteric streaks and hyper-echogenic serosal/omental infiltration; fistula and abscess). It can evaluate stenosis and its impact on transit from insignificant slowing to (sub-)ileus. In the differential diagnosis it will show the continuous echopoor wall transformation of severe bouts of ulcerous colitis, its upward extension, its remission and it will demonstrate the gyriform or accentuated wall thickening and neighbouring lymph node enlargement in infectious enterocolitis.
