Due à la prolifération d'endomètre fonctionnel en dehors de la cavité utérine, l'endométriose est une affection qui peut entraîner douleurs et infertilité. Le tube digestif est la plus fréquente des localisations extragénitales et les difficultés diagnostiques et thérapeutiques qu'entraînent de telles formes de la maladie doivent être bien connues des gastroentérologues, gynécologues, chriurgiens et internistes. Seule l'échoendoscopie rectale permet de déterminer avec une bonne fiabilité l'existence d'une localisation digestive, et de prévoir la difficulté de la résection cœlioscopique. Elle constitue donc, avec l'IRM, l'essentiel du bilan d'imagerie préopératorie. Au-delà de l'étude des parois digestives, l'écho-endoscopie par voie ano-rectale (EER), permet d'évaluer la vessie, les culs de sac vaginaux, l'utérus, le vagin et les ovaires. Ses deux principales indications en gynécologie sont l'étude des défects sphinctériens postobstétricaux, et la recherche des localisations digestives de l'endométriose. Une fois rappelés les signes et la pathogénie de celles-ci, nous décrirons la technique de l'EER, et l'écho-anatomie pelvienne qu'elle permet d'obtenir. La place respective de l'EER et des autres méthodes d'imagerie dans l'endométriose pelvienne et digestive, sera ensuite décrite.
Summary
Endometriosis is a pathology due to a proliferation of functional endometrium outside the uterine cavity mostly to the digestive tract. The clinical signs are pain and infertility. The diagnostic and therapeutic approach is sometimes defficult and therefore requires an interdisciplinary collaboration between internal medicine, gastroenterology, gynaecology and surgery. Rectal echoendoscopy provides not only a reliable demonstration of digestive sites of endometriosis but also has a good predictive value of possible difficulties during a coelioscopic resection. Associated with MRI, this technique is the best choice for an imaging check-up before surgery. Anorectal echoendoscopy (AREE) over the digestive wall appraisal gives a look about bordering organs such as bladder, vaginal pouches, vagina and moreover uterus and ovaries. The survey of post partum sphincter defects and the diagnosis of endometriosis located to the digestive tract are the two principal indications of AREE. After a short recall of the clinical signs and the pathogenesis of pelvic and digestive endometriosis we will give detailed information about the AREE method, pelvic echoanatomy and the place of this procedure related to other methods of imagery.
