Résumé
Le cancer du canal anal représente moins de 2% des cancers intestinaux. Depuis les années 80, le traitement de cette tumeur est conservateur. Actuellement, les formes peu invasives sont traitées par radiothérapie exclusive et les formes localement avancées par une radio-chimiothérapie. La chirurgie n'est plus indiquée qu'en cas d'échec du traitement médical ou en cas de récidive. Se pose donc le problème du bilan d'extension loco-régional pré-thérapeutique.
La classification des cancers du canal anal repose sur l'examen clinique et l'appréciation de la hauteur tumorale (classification UICC 1987). Cette classification a une application thérapeutique directe puisque les tumeurs classées T1 et T2 sont généralement traitées par une radiothérapie exclusive et les lésions classées T3, T4 ou N+ quel que soit le T relèvent d'une radio-chimiothérapie. Le but du travail prospectif multicentrique du CFED a été d'évaleur l'EEAR et de définir une classification us TN pronostique. 146 patients (42 hommes et 104 femmes d'âge moyen 63 ans) ont été inclus dans cette étude par 8 différents centres. La classification us TN apprécie le degré d'infiltration pariétale et l'extension ganglionnaire peri-rectale basse et haute (charnière) mais ignore l'atteinte ganglionnaire inguinale. Les tumeurs classées UICC T1 et T2 (< 4 cm) No ont été traitées par une radiothérapie exclusive, les lésions UICC T2 (> 4 cm), T3 et T4, No-N1-2-3 ont bénéficié d'une radiochimiothérapie concomitante. Résultats: Les données concernant la réponse au traitement et le suivi des patients (septembre 97) ont été disponibles chez 115/146 patients inclus (78,7%). Nous avons comparé l'impact pronostique des 2 classifications sur la réponse au traitement et sur la survenue d'une récidive loco-régionale (test chi-deux). Pour la réponse au traitement, 56/70 UICC T1-T2N0 (80%) ont eu une réponse complète, contre 34/36 usT1-2N0 (94,5%) [p=0,008]; par contre, le N évalué aussi bien cliniquement que par EEAR n'est pas pronostique. Pour le diagnostic de récidive, le T et le N évalués cliniquement ne sont pas pronostiques (respectivement p=0,4 et p=0,5); par contre usT et usN sont hautement prédictifs (respectivement p=0,001 et p=0,03). Conclusion: Les résultats préliminaires de cette étude multicentrique prospective montrent que le us T est prédictif de la réponse au traitement et que usT et usN sont prédictifs d'une récidive loco-régionale. L'étude devra être poursuivie pour apprécier le caractère pronostique de l'EEAR sur la survie.
Summary
The anal cancer represents less than 2% of the intestinal cancers. Since the 80s, the treatment of this tumour is conservative. At present, superficial lesions are treated by exclusive radiotherapy and locally advanced tumours with a radio-chemotherapy. Surgery is indicated only in case of failure of the medical treatment or in case of recurrence. So arises the problem of the loco-regional staging of extension.
The classification of the anal cancer bases on the clinical examination and the appreciation of the tumor height (classification UICC on 1987). This classification has a direct therapeutic application because the classified tumor T1 and T2 is generally treated by an exclusive radiotherapy and classified T3, T4 or N+recover from a radio-chemotherapy. The purpose of the multicentrique study of the CFED was to estimate the EEAR and to define an us TN classification. 146 patients (42 men and 104 women of average age 63 years) were included in this study by 8 various centres. The classification us TN appreciates the degree of wall infiltration and peri-rectal lymph node extension but ignores the inguinal lymph node involvement. The classified tumours UICC T1 and T2 (<4 cm) N0 were treated by an exclusive radiotherapy, UICC T2 (>4 cm). T3, T4 and N0-N1-2-3 benefited from a concomitant radio-chemotherapy. Results: data concerning response to the treatment and the follow-up of the patients were available in 115/146 included patients (78,7%). We compared impact preview of 2 classifications on the response to the treatment and on the presence of a loco-regional recurrence (test chi-2). For the response to the treatment, 56/70 UICC T1-T2N0 (80%) had a complete response, against 34/36 usTI-2N0 (94,5%) [p=0.008]; on the other hand, N estimated as very clinically by EEAR is not pronostic. For the diagnosis of recurrence, T and N estimated clinically is predictive (respectively p=0.4 and p=0.5); on the other hand, us T and usN are highly prognostic (respectively p=0.001 and p=0.03). Conclusion: the preliminary results of this multicentrique study show that the us T is prognostic of the response to the treatment and that us T and usN are prognostic of a loco-regional recurrence.
