Résumé
Il y a peu d’études dans la littérature rapportant les résultats de biopsies trans-rectales ou trans-coliques guidées par échoendoscopie.
On retrouve peu de cas de biopsie guidée pour des lésions péri-rectales ou péri-coliques dans 2 des plus grandes séries de la littérature de biopsie guidée sous EE. Dans la première regroupant 141 patients, seulement 7 biopsies (4,9 %) ont été réalisées pour une lésion périrectale ou péri-colique. Dans cette étude, la sensibilité de la biopsie guidée a été de 80 %. Dans la deuxième étude, 17 des 554 lésions biopsiées (3 %) selon cette technique siégeaient dans le pelvis ou l’abdomen. Le type histologique des ces lésions n’est pas rapporté dans l’article, par contre la sensibilité de la biopsie guidée sous EE était de 96 %.
En fait, plus que des taux de sensibilité ou de fiabilité, ce qui est important est de savoir si ce type de prélèvement dans ces indications modifie la conduite thérapeutique à tenir.
On peut répondre par l’affirmative lorsque l’échoendoscopie et la biopsie guidée confirme le diagnostic de récidive loco-régionale d’un cancer colo-rectal car seuls les patients pouvant bénéficier d’une exérèse complète pourront espérer une survie médiane à 5 ans (environ 30–35 %). En revanche, en ce qui concerne les tumeurs sous-muqueuses ou les compressions extrinsèques du rectum par des masses périrectales, le traitement de ces lésions est uniquement chirurgical et l’obtention d’une histologie préopératoire n’est pas nécessaire.
Faut-il réaliser une biopsie guidée en cas de tumeur superficielle du rectum (usT1 ou us T2) s’accompagnant d’une adénopathie périrectale ? Dans l’absolu, il faudrait répondre oui. En fait dans la pratique courante, nous avons tendance à considérer ces ganglions comme envahis. En effet, nous préférons réaliser une radiothérapie préopératoire qui est toujours très bien tolérée plutôt qu’une radiochimiothérapie postopératoire.
Enfin, lorsque l’échoendoscopie colique est le seul examen à mettre en évidence des nodules de carcinose péritonéale et si le patient est en bon état général, il est alors indispensable de réaliser une histologie de ce nodule afin de confirmer cette récidive. Ceci peut avoir une implication thérapeutique importante du fait des progrès de la chirurgie de la carcinose péritonéale d’origine colo-rectale.
Summary
There are few studies in the literature presenting results of transrectal or transcolic biopsies guided by echoendoscopy (EUS).
Few cases of guided biopsy for perirectal or pericolic lesions are reported in 2 of the largest series of EUS guided biopsy. In the first one concerning 141 patients, only 7 biopsies (4,9%) have been realized for perirectal or pericolic lesions. In this study, the sensitivity of the EUS guided biopsy was 80%. In the second study, 17 of 554 biopsied lesions (3%) according to this technique were located in the pelvis or the abdomen. The histology of these lesions is not mentioned in the article. The sensitivity of the EUS guided biopsy was 96%.
In fact, more than sensitivity or reliability rate, it is important to know if this type of biopsy in these indications modifies the therapeutic decision - making.
One can reply positively when the EUS guided biopsy confirms the diagnosis of relapse of a colorectal cancer because only patients able to benefit from a complete resection might expect a median survival of 5 years (30–35%). Concerning submucosal tumors or extrinsic compression of the rectum, the treatment of these types of lesions is only surgical and preoperative histology is not necessary.
Is it necessary to realize a EUS guided biopsy in case of superficial tumor of the rectum (usT1 or T2) with a perirectal lymph node? In the absolute, the answer is yes. In fact, in current practice, we consider these lymph nodes as malignant. Indeed, we prefer to realize a preoperative radiation therapy that is always very well tolerated rather than a postoperative radiochemotherapy.
Finally, when the colic EUS is the only examination to show nodules of peritoneal carcinomatosis and if the patient is in good condition, it is then indispensable to realize a biopsy of this nodule in order to confirm the relapse. This can have an important therapeutic implication due to the progress of the peritoneal carcinomatosis surgery.
