Résumé
Le Reflux Gastro-Œsophagien (RGO) est le passage du contenu gastrique, généralement acide, dans l’œsophage. Ce phénomène, tout à fait physiologique en post prandial, peut devenir pathologique par sa fréquence, sa durée et les manifestations cliniques qui en résultent. Ces dernières sont très évocatrices en présence de pyrosis et de régurgitations avec ou sans syndrome postural. Elles sont atypiques une fois sur deux et rendent le diagnostic plus délicat.
L’ambition des explorations fonctionnelles est de faire la part des choses tant sur le plan diagnostique que thérapeutique. Les tests actuellement disponibles sont nombreux, ce qui prouve leur insuffisance.
La démarche à adopter sera dictée par la clinique et la Symptomatologie d’appel. Dans tous les cas, deux explorations restent incontournables dans la prise en charge du RGO: la Fibroscopie Œso-Gastro-Duodénale (FOGD) et la pH-métrie avec ou sans manométrie œsophagienne (MO).
La multiplicité des moyens disponibles ne doit pas faire surestimer leur place qui sera précisée à chaque fois avec un souci d’efficacité et de moindre coût pour une bonne qualité de la vie.
Summary
The gastrooesophageal reflux disease consists of the flow of gastric contents, in general acidic, in the oesophagus. This phenomenon, which is truly physiological after meals, can become a pathology as a consequence of its frequency, its duration and the resulting clinical signs. The latter are very obvious in the case of pyrosis and régurgitations with or without postural syndrome. In one case out of two, they are not typical and render the diagnosis more difficult.
The aim of functional explorations is to sort out things both from a diagnostic and therapeutic point of view. There are currently many tests, this abundance showing their inefftcacy.
The procedure to follow will be guided by the clinical symptoms. In all cases, two explorations remain indispensable in the management of GOR: the oesogastroduodenal fibroscopy (OGDF) and pH-metry with or without oesophageal manometry.
The high number of the available tests should not lead to overestimate their value which will be defined every time with efficacy and cost consciousness while preserving a good quality of life.
