Résumé
200 œsophagoscopies avec ou sans dilatation ont été faites sous anesthésie générale chez 20 enfants de 2 mois à 10 ans pour les affections suivantes:
-
-brûlure du tractus digestif de la bouche à l'estomac,
-
-sténose œsophagienne sur brûlure ou sténose de la suture après chirurgie de l'œsophage,
-
-sténose œsophagienne sur herpès, radiothérapie, œsophagite peptique.
Les examens se passent en salle d'opération sous monitoring cardio-respiratoire continu.
La technique de narcose est intraveineuse, au moyen de méthohexidal, thalamonal et succinylcholine ou en intramusculaire avec de la kétamine, suivie par la voie intraveineuse.
On intube toujours la trachée; le tube est stérile, en polyéthylène. Il n'y a pas eu de lésions de la glotte avec ce matériel, malgré les intubations multiples.
Il y a eu 3 perforations de l'œsophage chez les brûlés.
Un enfant a présenté au réveil. un syndrome extrapyramidal dû au thalamonal et réversible par la ponalide.
En postopératoire, la surveillance pulmonaire est indispensable pour déceler un pneumothorax éventuel ou un foyer pulmonaire.
Summary
200 oesophagoscopies with or without dilatation were performed under general anesthesia in 20 children for the following conditions:
-
-burn of the digestive tract from mouth to stomach.
-
-stenosis of the oesophagus due to burn or surgical suture,
-
-stenosis of the oesophagus due to herpes, radium therapy or peptic oesophagitis.
Examinations are performed in the operating room with continuous cardiorespiratory monitoring.
The technique of anesthesia consists of either intravenous injection of methohexital, thalamonal and succinylcholine or intramuscular injection of ketamine followed by its intravenous injection.
Trachea is always intubated with a sterile polyethylene endotracheal tube.
There was no damage to the glottis with these tubes, despite of multiple intubations.
There were 3 perforations of the oesophagus in the burned patients.
A child developed an extrapyramidal syndrome upon awakening due to thalamonal, which was successfully treated with ponalide.
During the postoperative period, pulmonary examination is mandatory for the detection of pneumothorax or of lung infection.
