Résumé
La gastrite lymphocytaire est une entité histopathologique se caractérisant par une importante infiltration lymphocytaire de l’épithélium gastrique; elle est en relation étroite avec un aspect endoscopique défini sous le nom de gastrite varioliforme diffuse.
Nous avons étudié par méthode immunocytochimique à la Peroxydase les lymphocytes intra-épithéliaux et choriaux présents dans cette affection. Nous avons montré que les lymphocytes intra-épithéliaux sont tous de type ? avec une nette prédominance des lymphocytes T8 sur les lymphocytes T4 tandis que le chorion est le siège d’un infiltrat lymphocytaire beaucoup moins marqué avec légère prédominance de T4 par rapport au T8; les lymphocytes sont peu nombreux.
La composition de l’infiltrat lymphocytaire varie peu au cours des phases de résolution de la maladie.
Summary
Lymphocytic gastritis is characterised by a heavy intraepithelial lymphocytic infiltration with endoscopic appearances of diffuse varioliform gastritis in the majority of cases. We have studied the immunocytochemical profile of the lymphocytes in this disorder on frozen tissue from 25 patients seen at different stages of severity of the disease. All intraepithelial lymphocytes were of ? lineage with a clear predominance of the T8 (suppressor/cytotoxic) subgroup. B cells were not seen in the epithelium and were rare in the lamina propria. Numerous T8 (suppressor/cytotoxic) lymphocytes and T4 (helper) lymphocytes were present in the lamina propria, with a slight predominance of T4 lymphocytes.
Analogies between the present findings and those described in coeliac disease suggest an abnormal response to some local antigen as an aetiological factor in lymphocytic gastritis.

La gastritis linfocitaria es una entidad histopatológica que se caracteriza por una importante infiltración linfocitaria del epitelio gástrico; está en intima relación con un aspecto endoscópico bien definido, conocido como gastritis varioliforme difusa.
Hemos estudiado los linfocitos intraepiteliales y del corion presentes en dicha enfermedad, por el método inmunocitoquimico de la peroxidasa. Hemos podido comprobar que los linfocitos intraepiteliales son todos del tipo ? con un claro predominio de los linfocitos T8 sobre los T4, mientras que en el corion el infiltrado es menos denso, con una ligera predominancia de los T4 sobre los T8; los linfocitos aparecen en poca cantidad.
La composición del infiltrado linfocitario varia poco en los periodos de resolución de la enfermedad.
