Le vidéoendoscope électronique couplé à un détecteur monochromatique (CCD-chip pour Charge Coupled Device) permet la détermination du contenu en hémoglobine de la muqueuse par analyse d’image. La lumière de l’endoscope est composée de trois couleurs primaires (rouge, vert et bleu). L’hémoglobine est le pigment dominant de la muqueuse du tube digestif. L’hémoglobine absorbe la lumière verte, mais réfléchit la plus grande partie de la lumière rouge, rendant ainsi possible l’estimation du contenu en hémoglobine de la muqueuse en utilisant la différence d’intensité lumineuse des images rouge et verte. L’étude de quinze patients présentant un œsophage macroscopiquement normal et dix patients avec une œsophagite macroscopique démontre la faisabilité d’une estimation par ordinateur du contenu en hémoglobine de la muqueuse grâce à l’analyse de l’image endoscopique avec un programme spécialement conçu à cet effet. L’estimation par ordinateur du contenu en hémoglobine de la muqueuse (index Hb) révèle que celui-ci est significativement élevé au niveau des zones inflammées par rapport à la muqueuse normale.
Summary
Electronic Videoendoscopy operating a monochrome Charge Coupled Device (CCD-chip) enables analysis of the mucosal hemoglobin content by image processing. Light from the endoscope is composed of the three primary colours (red, green and blue). Haemoglobin is the most dominant pigment in the mucosa of the gastrointestinal tract. Haemoglobin absorbs the green light, but reflects most of the red light, so it is therefore possible to estimate the haemoglobin content in the mucosa using the difference in light intensity of the red and green image. The study of fifteen patients with macroscopically normal esophagus and ten patients with macroscopically esophagitis demonstrates the feasibility of the computerised estimation of the mucosal hemoglobin content by means of image processing of the endoscopic image with a special designed software. The computerised estimation of the mucosal hemoglobin content (Hb-index) reveals that the Hb-index is significantly elevated in the areas with inflammation compared to the normal mucosa.
