Résumé
Depuis quelques années, le laser est utilisé à titre palliatif dans le traitement des lésions tumorales sténosantes du tractus digestif supérieur. Cette étude multicentrique regroupe les données du traitement de 326 malades qui ont reçu un total de 2 000 séances de laser.
Nous avons observé un taux global de complications de 14,3 % soit 49 complications directement imputables à la photocoagulation laser. La complication la plus fréquemment observée est la perforation médiastinale, rencontrée chez 4,2 % des patients et survenant au cours de 0,7 % des séances. Les hémorragies viennent ensuite, survenant chez 3 % des malades au cours de 0,5 % des séances. Les sténoses cicatricielles se rencontrent après traitement d’une lésion circonférencielle, dans 2,6 % des cas. Deux cas de fistule œso-trachéale ont été observés.
La fréquence des complications semble influencée par la localisation de la lésion : les malades porteurs de lésions du tiers supérieur ou moyen de l’œsophage présentent plus de complications que les autres. De même, les malades qui ont subi des séances de radiothérapie avant le laser présentent un taux de complications trois fois supérieur à celui de l’ensemble de la population.
34 % des malades présentant une complication ont nécessité un traitement chirurgical et 8 malades sont décédés soit 4,9 % de l’ensemble des malades traités.
Le taux de complications observées après photocoagulation par laser apparaît donc inférieur à celui du traitement chirurgical palliatif (environ 25 %) et inférieur également à celui d’autres méthodes endoscopiques tel la pose de prothèse (15 à 35 %).
Summary
In the past few years, laser has been used as palliative treatment of stenosing tumoral lesions of the upper digestive tract. This multi-center study is based on treatment of 326 patients that underwent a total of 2000 laser sessions.
The global rate of complications was 14.3 % or in other words 49 complications which were directly caused by laser photocoagulation. The most frequent complication is mediastinal perforation which was found in 4.2 % of patients, occurring in 0.7 % of the laser sessions. Then follows hemorrhage which occurs in 3 % of patients in 0.5 % of laser sessions. Cicatricial stenosis are found after treatment of a circumferential lesion, in 2.6 % of cases. 2 cases ofeso-tracheal fistula were also observed.
The frequency of complications is influenced by the localization of the lesion : those patients with lesions of the upper and middle esophagus have more complications than the others. Also patients that had undergone radiotherapy before laser therapy have 3 times more complications than the others.
34 % of patients with complications needed surgical treatment and 8 patients died or in other words 4.9 % of all the patients treated.
There are fewer complications arising from laser photocoagulation compared to palliative surgical treatment (about 25 %) as well as compared to other endoscopic methods such as putting a prosthesis into position.
