Résumé
Les lésions endoscopiques et la distensibilité de l’ampoule rectale après colectomie et anastomose iléo-rectale (AIR) pour rectocolite hémorragique (RCH) ont été étudiées chez deux groupes de sujets :

1)
27 patients porteurs d’une AIR après colectomie pour RCH, la durée moyenne d’observation après AIR était de 5 ans (4 mois-17 ans), l’ancienneté de la maladie était en moyenne de 11 ans et demi (1 an-47 ans).

2)
9 patients atteints de RCH non opérée, appariés pour l’âge et le sexe avec les patients colectomisés et dont la maladie évoluait depuis 4 ans et demi en moyenne (2 mois-16 ans).
Ces deux groupes de sujets ont été comparés entre eux ð un troisième groupe comprenant 15 sujets normaux ayant une selle par jour et ne prenant aucun traitement. La distensibilité rectale a été évaluée d’après les résutats de la mesure de la capacité rectale (volume de perception constante (VPC) et volume maximum tolérable (VMT) établis selon la méthode de Farthing et Lennard Jones) et du coefficient d’élasticité rectale (Eα) qui varie en sens inverse de la distensibilité de la paroi rectale.

La capacité rectale (VPC et VMT) des 27 AIR est comparable ð celle des sujets normaux mais la capacité rectale des RCH non opérées avec rectum de stade II-III (n = 8) est significativement plus basse que celle des sujets normaux (P < 0,01). La capacité rectale (VPC et VMT) des AIR avec rectum de stade II-III (n = 21) est significativement supérieure (P < 0,05) ð celle des RCH non opérées de même stade endoscopique.

Les valeurs de E » des 27 AIR sont comparables ð celles des sujets normaux.
Les valeurs de E » des RCH non opérées (n = 8) avec rectum de stade évolutif sont non seulement supérieures ð celles des sujets normaux (P = 0,09) mais également ð celles des AIR de même stade endoscopique (n = 21) (P = 0,1). La compliance rectale des AIR avec rectum de stade II-III est donc supérieure ð celle des RCH non opérées avec rectum de même stade.

II existe une corrélation hautement significative entre les valeurs du VMT et celles de E » mesurées chez les 27 AIR (P < 0,001). Ces résultats suggèrent qu’il existe une capacité d’adaptation fonctionnelle de la paroi rectale après AIR pour RCH même pour les formes endoscopiquement sévères.
Summary
The endoscopic lesions and the distensibility of the rectal ampulla after colectomy and ileo- rectal anastomosis (IRA) for ulcerative rectocolitis was studied in 2 groups of patients :

27 patients with IRA after colectomy for ulcerative colitis (UC), the average length of observation after the IRA was 5 years (between 4 months and 17 years), and the average length of time from the beginning of the illness was 11 and a half years (1 year- 47 years),

9 patients with an unoperated UC, matched as to age and sex with the colectomized patients and whose illness had been developing on average for 4 and a half years (2 months-16 years).
These 2 groups of patients were compared between themselves and a third controlled group with 15 normal people having 1 stool per day and undergoing no treatment. The distensibility of the rectum was evaluated according to the results of the measurement of the capacity of the rectum (volume of constant perception (VCP) and the maximum tolerable volume (MTV) establihed by Farthing and Lennard Jone’s methods and the coefficient of rectal elasticity (E » @#@) which varies inversely to the distensibility of the rectal wall.

The rectal capacity (VCP and MTV) of the 27 IRA was comparable to that of the normal people from the controlled group but the rectal capacity of the unoperated UC with rectum at stage II- III (n = 8) was significantly lower than of the normal patients (P 0.01). The rectal capacity (VCP and MTV) of the IRA with rectum at stage II- III (n = 21) was significantly higher (P < 0.05) than that of the unoperated UC at the same endoscopic stage.

The figures of E » of the 27 IRA were comparable to those of the controlled group. The figures of E » of the non operated UC (n = 8) with a rectum at a developing stage were not only higher than those of the controlled group (P = 0.09) but also than those of the IRA at the same endoscopic stage (n = 21) (P = 0.1). The rectal elasticity of the IRA with a rectum at stage II- III is thus higher to that of the unoperated UC with a rectum at the same stage.

There exists a highly significant correlation between the figures of MTV and those of E » measured in the 27 IRA (P < 0.001).
The results suggests that there exists a capacity of functional adaptation of the rectal wall after IRA for UC even for the most endoscopically serious forms.
