La pHmétrie œsophagienne ne peut mettre en évidence les accès de reflux gastro-œsophagien (inférieurs à pH 4) que si le contenu gastrique est suffisamment acide (pH < 4).
Un double enregistrement, simultané, des pH œsophagien et gastrique a été réalisé chez 129 enfants. Les périodes pendant lesquelles le pH gastrique était supérieur à 4 ont été exclues de l’interprétation.
Nous avons comparé le pourcentage de temps pendant lequel le pH œsophagien se situait en dessous de 4 par rapport au temps total d’enregistrement et par rapport au temps corrigé: 30 pHmétries sur 129 (soit 23,3 %) ont vu leur conclusion grandement modifiée.
Ainsi, en tenant compte du tamponnement de l’acidité gastrique, nous avons pu augmenter nettement la sensibilité de la pHmétrie œsophagienne.
Summary
Oesophageal pHmetry cannot reveal bouts of gastro- œsophagealreflux (lower than pH 4) unless the gastric content is sufficiently acidic (pH < 4).
A simultaneous recording of the œsophageal and gastric pH was carried out in 129 children. The intervals when the gastric pH was higher than 4 were not included in the analysis of results.
We have compared the percentage of the time when the œsophageal pH was higher than 4 with the total time of recording as well as with the corrected time, which saw the final result of 30 pH recordings out of the 129 (i.e. 23.3 %) considerably modified Thus, by taking into account the buffering action of gastric acidity, we have been able to improve considerably the sensitivity of œsophageal pHmetry.

La pH- metrìa esofágica no puede poner en evidencia los accesos de reflujo gastroesofágico (inferior a pH 4) sino cuando el contenido gástrico es suficientemente ácido (pH 4).
Un doble registre, simultánee del pH esofágico y gástrico ha sido realizado en 129 niños. Los períodes durante los cuales el pH gástrico era superior a 4 se han excluido de la interpretación.
Hemos comparado el porcentaje del tiempo durante el cual el pH esofágico se sitúa por debajo de 4 en relación al tiempo total de registre y en relación al tiempo corregido: 30 pH- metrías sobre 129 (23.3 %) han visto su conclusion grandemente modificada.
Así, teniendo en cuenta el tamponamiento de la acidez gástrica hemos podido aumentar netamente la sensibilidad de la pH- metría esofágica.
Zusammenfassung
Die pH- Messung im Bereich des Ösophagus kann nur einen gastro- ösophagealen Reflux (mit einem pH < 4) nachweisen, wenn der Mageninhalt ebenfalls ausreichend sauer ist (pH < 4).
Eine gleichzeitige Registrierung des pH- Wertes im ösophagus und im Magen wurde bei 129 Kindern durchgeführt. Die Perioden, in denen der pH im Magen über 4 lag, wurden aus der Studie ausgeschlossen.
Wir haben den prozentualen Anteil der Zeit, in der Ösophagus- pH unterhalb 4 lag, mit der Gesamtregistrierungszeit und mit einer korrigierten Zeit verglichen. 30 Untersuchungen von 129 (d.h.23, 3 v.100) sind so erheblich modifiziert worden.
So konnte unter Berücksichtigung der Magensaüre die Sensibilität der Ösophagus- pH- Metrie erheblich gesteigert werden.

La pH- metria esofagea non è in grado di evidenziare episodi di reflusso gastro- esofageo (inferiore a pH 4) in presenza di contenuto gastrico non sufficentemento acido (pH < 4).
Una doppia registrazione simultanea del pH, esofageo e gastrico, èstatu effettuata in 129 bambini. I periodi nei quali il pH dello stomaco era superiore a 4 sono stati esclusi dallo studio. Sono state confrontate le percentuali di tempo durante le quali il pH esofageo era inferiore a 4 in rapporte al tempo totale di registrazione ed al tempo corretto: in 30 pH- metrie su 129 (23.3 %) ciòha portato a modificazioni rilevanti delle conclusioni. In questa maniera, tenendo conto del tamponamento dell’aciditàgastrica, abbiamo ottenuto un aumento rilevante della sensibilitádella pH- metria esofagea.
