Résumé
La première hémorragie digestive par rupture de varices œsophagiennes marque un tournant dans l’évolution de l’hypertension portale et donc de la cirrhose. L’histoire naturelle du développement des varices œsophagiennes est actuellement assez bien connue. Dans l’avenir, le développement de programmes thérapeutiques de prophylaxie primaire et pré-primaire risque de freiner l’acquisition de nouvelles connaissances dans ce domaine. Lors de l’examen endoscopique, il faut évaluer à la fois: la taille des varices et la présence de « signes rouges ». En effet, le risque hémorragique en est dépendant. Le traitement endoscopique pourra également être modulé en fonction de ces paramètres. La présence ou non de varices gastriques est également importante pour le traitement préventif ou curatif. Il faut en effet se souvenir que la mortalité est de 30% environ lors du premier épisode hémorragique et augmente encore en cas de récidive précoce (moins de 10 jours). Le traitement initial est donc très important. Le pronostic immédiat et à distance est également dépendant de la gravité de la cirrhose et donc du score de Child Pugh.
Summary
The first hemorrhage due to gastric or oesophageal varices is a very important event in patients with hepatic cirhosis. Natural history of oesophageal varices is quite well known. In the future, the development of preventive treatment will decrease the possibility to increase our knowledge of this field. At endoscopic examination, either for the first bleeding or in a patient with cirrhosis, but who never bled, it is very important to evaluate the size of oesophageal, but also of gastric varices, and to quantify the importance of red signs. This will help for therapeutic choices. It is important to point out that the mortality induced by the first bleeding is of about 30%. Mortality is also dependant on the severity of hepatic insufficiency as measured by Child Pugh index.
