Résumé

Nous avons réalisé une étude histologique comparative de différents types de colites afin ďévaluer ľintérêt du granulome et du microgranulome dans le diagnostic de la maladie de Crohn. Nos résultats confirment ľimportance du granulome que nous n’avons pas retrouvé en dehors de la colite granulomateuse. Par contre, ils mettent en doute la spécificité du microgranulome. En effet, si nous avons rencontré ce dernier dans 22% des colites granulomateuses, nous ľavons également trouvé dans 4% des colites aspécifiques, 7% des rectocolites ulcéro-hémorragiques et 13% des colites infectieuses.

Il paraît exister des variations dans ľarchitecture et la composition cellulaire des microgranulomes. Les différences histologiques sont cependant difficilement perceptibles lors ďun examen de routine et ne sont pas utilisables en pratique courante.

Summary

We realized a comparative histologic study of various types of colitis in order to evaluate the interest of the granuloma and microgranuloma in the diagnosis of colonic Crohn’s disease. Our results confirm the diagnostic importance of the granuloma which was not found outside the granulomatous colitis. But they cast some doubt on the specificity of the microgranuloma that we found not only in Crohn’s disease (22%) but also in aspecific (4%), ulcerative (7%) and infective (13%) colitis.

Variations seem to exist in the architecture and cell composition of the microgranulomas. However, histologic differences are not characteristic of one disease and are hardly perceptible at routine examination.
