La laparoscopie dans le diagnostic des maladies du pancréas

Résumé

Jusqu’il y a quelques années, le pancréas était considéré comme inaccessible aux méthodes d’exploration classiques et sa biopsie était regardée comme extrêmement dangereuse en raison du risque d’hémorragie, de fistule, de pancréatite, etc.

Récemment une série de méthodes ont été développées, rendant possible une excellente étude de son parenchyme et la réalisation de biopsies pancréatiques avec un faible risque.

Cet article présente les méthodes laparoscopiques. 119 patients ont été étudiés: 100 avaient un pancréas normal, six un cancer du pancréas, sept une pancréatite chronique, quatre un faux kyste pancréatique et deux ont été examinés en cours de pancréatite aiguë.

Bien que cette méthode d’exploration doive être utilisée après les résultats infructueux de méthodes non-invasives, les auteurs soulignent son utilité dans le diagnostic des maladies pancréatiques et en particulier l’absence de complications après biopsies du pancréas.

Summary

Up to a few years ago, the pancreas was thought to be almost inaccessible to our explorations methods and its biopsy was extremely dangerous because of the risk of bleeding, fistulas, pancreatitis, etc.

Recently, a series of methods have emerged, that have made possible not only an excellent study of the viscera but have also shown a low risk when carrying out a pancreatic biopsy.

In this article, the laparoscopic methods are described. One hundred and nineteen patients have been studied, a hundred having normal pancreas, six with pancreatic cancer, seven with chronic pancreatitis, four with a pancreatic cyst and two with acute pancreatitis.

Although the authors consider that this method of exploration should only be employed when a correct diagnosis cannot be arrived at by other non-invasive methods, they would like to emphasize its usefulness in the diagnosis of pancreatic diseases and the absence of complications when carrying out pancreatic biopsies.
