Résumé
Les sels biliaires sont considéréd comme agents pathogènes dans la gastrite par reflux, y compris chez les malades non soumis à une chirurgie gastrique. L’effet nuisible sur la muqueuse gastrique differe d'un sel biliaire à I’autre, et I’acide ursodeoxycholique (UDCA) est considéré comme moins toxique.
Trente patients présentant une gastrite superficielle par reflux alcalin ont été soumis de façon randomisée, à un traitement par UDCA (150 mg deux fois par jour) versus un placebo administré pendant 12 semaines au cours d’un essai contrôlé en double insu. La teneur totale en sels biliaires du sue gastrique a été mesurée afin d’établir l'existence déun reflux duodéno-gastrique. La séverité des symptômes dyspeptiques et les degrés de sévérité, endoscopiques et histologiques, de la gastrite ont été évalués avant le traitement et à la fin de l’étude.
Aucune différence n’a été observée entre le traitement par I’UDCA et celui par placebo, en ce qui concerne les effets sur les symptômes, sur les données endoscopiques et histologiques. Néanmoins, un effet statistiquement significatif a été observé, tant pour I’UDCA que pour le placebo, en comparant d’une part les scores symptômatiques à 6 et 12 semaines, les aspects endoscopiques à la douzieme semaine, et d’autre part ceux correspondant à la période pré-thérapeutique.
L’important “ effet placebo ” constaté au cours de notre étude ne permet pas d’aboutir à des résultats concluants. La démonstration d’un éventuel avantage de I’UDCA dans le traitement de la gastrite chez les malades non soumis à la chirurgie gastrique, nécessiterait l’élargissement de l’étude à un plus grand nombre de cas.
Summary
Bile acids have been incriminated in the pathogenesis of reflux gastritis, also in patients not submitted to gastric surgery. Their damaging effect on the gastric mucosa is different, UDCA being less noxious than other bile acids.
Thirty patients with superficial alkaline reflux gastritis were randomly treated with ursodeoxycholic acid (UDCA) (150 mg b.i.d.) or placebo for 12 weeks in a double-blind controlled trial. Total bile acids in the gastric samples were measured to confirm duodenogastric reflux. The severity of dyspeptic symptoms and the grade of endoscopic and histological gastritis were assessed before treatment and at the end of the study.
No differences were found between UDCA and placebo as for their effects on symptoms, endoscopic and histological features. However a statistically significant effect of both UDCA and placebo was found when clinical reports at 6 and 12 weeks and endoscopic and histological features at 12 weeks were compared with the corresponding pre-treatment findings.
The considerable “ placebo effect ” observed in our study makes the results inconclusive. Whether UDCA would be beneficial in the treatment of reflux gastritis in patients not submitted to gastric surgery needs further evaluation in larger series of subjects.
Resumen
Las sales biliares están consideradas como agentes patógenos en la gastritis por reflujo biliar, aún en pacientes no sometidos a Cirugía gástrica previa. El efecto nocivo sobre la mucosa gástrica difiere de una sal biliar a otra, considerandose el menos tóxico el ácido ursodeoxicólico (UDCA).
En un estudio controlado doble ciego se sometió, randomizandolos, a un grupo de 30 pacientes a un tratamiento con UDCA (150 mgs 2/veces-día) frente a un placebo. Dicho tratamiento se estableció durante 12 semanas. Con el fin de comprobar la existencia de reflujo duodenogástrico se analizó el contenido total de sales biliares en el jugo gástrico. Se valoró la gravedad de los síntomas dispépticos y los grados de severidad endoscópica de la gastritis, asi como de la afectación histológica, antes del tratamiento y al finalizar el estudio. No se pudo demostrar ninguna diferencia entre el tratamiento con UDCA y el placebo, ni en la sintomatología, ni en los datos endoscopicos e histológicos. Sin embargo, si se demosyro un efecto estadisticamente significativo, tanto para el UDCA como para el placebo, si se comparaba, por una parte los “ scores ” sintomáticos a 6 y 12 semanas, los aspectos endoscopicos a las 12 semanas, y por otra parte los que correspondían al período pre-tratamiento. El importante efecto “ placebo ” observado en el curso de nuestro estudio no permite obtener resultados concluyentes. La demostración de una eventual ventaja del UDCA en el tratamiento de la gastritis, en pacientes no sometidos a Cirugía gástrica precisaría de una ampliación del trabajo a un número mayor de casos.
