Zusammenfassung
Grundlagen:
Ziel dieser Studie war die Untersuchung der Reperfusion unter dem Einfluß des ACE-Hemmers Ramiprilat nach 60 min Ischämie. Diese Studies sollte zeigen, ob durch Ramiprilat—in der Reperfusion appliziert—die bekannten antiischämischen Effekte der ACE-Hemmer zu beobachten sind. Bestimmt wurden präund postischämische hämodynamische und biochemische Parameter.
Methodik:
Die Untersuchungen wurden an einem erythrozytenperfundierten «Working-heart-Modell” durchgeführt. Die Versuche durchliefen folgendes Studienprotokoll: 15 min Langendorff-Modus (LDI), 30 min Working-heart-Modus (WHI), 60 min kalte Ischämie, 15 min LD2-Perfusion, 30 min WH2-Modus. Während der Ischämie wurde mit Blutkardioplegie nachBuckberg kardioplegiert. Als Perfusat diente eine Rindererythrozytensuspension. 9 Versuchstiere (Elco-Kaninchen) bildeten die Kontrollgruppe, 9 die Ramiprilatgruppe (2,58×10−6 mol/l).
Ergebnisse
In der Ramiprilatgruppe war die Herzfrequenz gegenüber der Kontrollgruppe signifikant niedriger (166±11 vs. 204±10, p<0,01). Weiters zeigte sich eine deutlich geringere Laktatkonzentration im Koronareffusat der Ramiprilatgruppe am Ende der Reperfusion (0,7 mmol/l vs. 1,0 mmol/l, p<0,01) sowie eine ebenfalls signifikante Erhöhung der GTP-Konzentration in dieser Gruppe (0,15 μmol/g Feuchtgewicht vs. 0,1 μmol/g, p<0,05). Außerdem konnte unter Ramiprilat die Arrhythmieinzidenz signifikant reduziert werden (11% Arrhythmien vs. 44%. p<0,01).
Schlußfolgerungen
Mit dieser Studie konnte gezeigt werden, daß sich der Einsatz des ACE-Hemmers Ramiprilat in der Reperfusion kardioprotektiv auswirkt. Dies gelingt über eine tendenzielle Erhöhung des Koronarflusses und eine bessere Präservation energiereicher Phosphate sowie eine signifikante Reduktion der Herzfrequenz, der Laktatproduktion und Arrhythmieinzidenz.
Summary
Background
The aim of this study was to determine the influence of the ACE-inhibitor ramiprilat on the reperfusion after 60 min cold ischemia, especially, whether ramiprilat provokes the well known antiischemic effects after application during the reperfusion. Pre- and postischemic hemodynamic and biochemical parameters were measured.
Methods
In our experiments a bovine-erythrocyte-perfused working-heart model was used. All hearts passed the same protocol: 15 min of Langendorff mode (LD1), 30 min of working-heart mode (WH1), 60 min of hypothermic cardiac arrest (16°C), 15 min of Langendorff mode (LD2) and 30 min of working-heart mode (WH2). During ischemia blood cardioplegia according toBuckberg was administered. 9 adult Elco-rabbits were representing the control group, 9 animals received ramiprilat during the reperfusion (2.58×10−6 mol/l).
Results
Heart rate was significantly lower in the ramiprilat-group compared with the control-group (166±11 vs. 204±10, p<0.01). At the end of reperfusion the concentration of lactate in the coronary effusate was significantly reduced in the ramiprilat-group (0.7 mmol/l vs. 1.0 mmol/l, p<0.01). Furthermore the GTP-level was significantly increased in the treatment-group compared with the control-group (0.15 μmol/g wet weight vs. 0.1 μmol/g, p<0.05). The incidence of ventricular fibrillation was significantly lower during the reperfusion in the ramiprilatgroup (11% vs. 44%, p<0.01).
Conclusions
These results indicated that ramiprilat administered during the reperfusion has cardioprotective properties by increasing coronary flux and the preservation of high energy phosphates slightly and by lowering the incidence of ventricular arrhythmias, lactate production and heart-rate.
