Zusammenfassung
Grundlagen
Es wird ein kurzgefaßter Rückblick über die 150jährige historische Entwicklung der Methode des isoliert perfundierten Warmblüterherzens gegeben, angefangen 1846 mit der parabiotischen KoronarperfusionCarl Ludwigs, überNewell Martins Herz-(Lungen-)Präparat bis hin zuOscar Langendorffs genial einfacher. völliger Isolierung und Perfusion des Herzens 1895.
Mit modernen elektronischen, Computer-gestützten Meß, Verrechnungs-und Registriermethoden ließ sich in den letzten Jahrzehnten—bei gleichgebliebener Präparationstechnik—der Informationsgewinn aus der Methode nach Langendorff erheblich steigern.
Methodik
Anhand eigener Experimente mit isolierten Meerschweinchenherzen wird exemplarisch die Konstruktion von Funktionskurven aus dem LV-Druck, der maximalen Druckanstiegs-und Druckabfallsgeschwindigkeit (+dp/dtmax, −dp/dtmax) und dem Koronarfluß—alle in Abhängigkeit vom Perfusionsdruck (PP)—demonstriert. Auf diese relativ einfache Weise kann das inotrope und lusitrope Verhalten des Myokards bei PP-Belastung sowie die myogene Autoregulation (MA) der Koronargefäße simultan objektiviert werden. Der Quotient (Q) [−dp/dtmax]/[+dp/dtmax] läßt Unterschiede im Verhältnis der beiden Geschwindigkeitsparameter des linken Ventrikels frühzeitig erkennen.
Ergebnisse
Bei Kontrollherzen und bei Perfusion mit dem Ca-Antagonisten Gallopamil (10−8 mol/l) bleibt Q bei steigender PP-Belastung konstant bei 0,8; Gallopamil wirkt jedoch stark negativ inotrop und hebt gleichzeitig die MA der Koronargefäße auf. Bei hypoxischer Perfusion nimmt dagegen Q als Ausdruck frühzeitiger Verminderung von −dp/dtmax zunächst um 38% ab, um bei höheren PP-Drucken, wegen der verzögert einsetzenden Abnahme von +dp/dtmax, wieder anzusteigen. Der Effekt auf Inotropie und MA ist etwa gleich wie unter Gallopamil. Die bekannte gestörte Ca++-Sequestration bei Hypoxie dürfte die Ursache für diese Beobachtung sein.
Schlußfolgerungen
Die beschriebene Art der Auswertung der belastungsbezogenen Meßparameter am isoliert perfundierten Herzen nach Langendorff eignet sich zur simultanen Auffindung von systolischen und diastolischen Dysfunktionen des Myokaids sowie zur Überprüfung der Koronargefäßmuskulatur.

Background
A short review is given over the 150 years historical development of the isolated perfused mammalian heart technique, starting in 1846 with the parabiotic coronary perfusion ofCarl Ludwig, continuing withNewell Martin’s heart (lung-) preparation and finally reachingOscar Langendorff’s ingeniously simple total isolation and perfusion of the heart in 1895.
Using modern electronic, computer-assisted methods of measuring, calculation and recording the gain of information from the method could be increased considerably during the last decades, while the technique of preparation remained unchanged.
Methods
Using our own experiments with isolated guinea pig hearts serving as examples the construction of function curves from LV pressure, maximum rate of pressure increase and pressure decrease (+dp/dtmax, −dp/dtmax) and coronary flow—all as a function of perfusion pressure (PP)—is demonstrated. This procedure allows for simultaneous objectivation of the inotropic and lusitropic behaviour of the myocardium under PP load as well as the myogenic autoregulation (MA) of the coronary vessels. By calculating the ratio (Q) [−dp/dtmax: +dp/dtmax] differences in both rate parameters of the left ventricle can be recognized early.
Results
In control hearts and during perfusion with the Ca antagonist gallopamil (10−8 mol/l) “Q” keeps stable at 0.8 at increasing PP load. However, gallopamil exerts a strong negative inotropic effect and simultaneously abolishes MA of the coronary vessels. During hypoxic perfusion “Q” initially decreases by 38% as a result of early reduction of −dp/dtmax and at higher PP increases again due to the delayed onset of +dp/dtmax decrease. The effect on inotropism and MA is equal as under gallopamil. The well-known disturbed Ca sequestration during hypoxia might be the cause for this observation.
Conclusions
The described evaluation of the load-related measuring parameters at the isolated perfused heart according to Langendorff is thus suited for the simultaneous detection of systolic and diastolic dysfunctions of the myocardium as well as for the examination of the coronary vasculature.
