Zusammenfassung
Phasenkontrast- und elektronenmikroskopische Untersuchungen an Gewebsproben von 18 Patienten über die morphologischen Veränderungen des menschlichen Gehirns nach intravenöser Harnstoffgabe.
Im Phasenkontrastmikroskop erscheint die Hirnrinde nach Harnstoff deutlich kompakter als normalerweise. Dies beruht, wie die elektronenmikroskopischen Befunde zeigen, auf einer Entquellung von Gliazellen und ihren Fortsätzen, wobei vorwiegend die Oligodendroglia betroffen ist. Die Oligodendrogliazellen sind unter Harnstoff dichter und kleiner, während die Astroglia unverändert erscheint. Die marginale Glia war unter Harnstoff ebenfalls deutlich geschrumpft.
Bei Patienten mit Großhirngliomen fanden sich in der Hirnrinde große perivasculäre Räume, wahrscheinlich bedingt durch Ruptur gequollener Astrocytenfortsätze (Hirnödem). Nach Harnstoff waren derartige perivasculäre Exsudate wesentlich seltener und geringer.
Sichere Veränderungen an der weißen Substanz waren nicht festzustellen. Dies ist wahrscheinlich auf die Gefäß- und Gliaarmut des Marks zurückzuführen.
Summary
The authors describe a phasecontrast- and electron-microscope investigation of the morphological changes in human brains after intravenous urea based on needle biopsy in 18 patients.
In phase contrast microscopy it appears that the cortex after urea becomes definitely more compact than normal. This is due, as the electron microscope shows, to an outflow from the glial cells and their processes with the oligodendroglia being predominantly affected. The oligodendroglia are denser and smaller after urea, whilst the astrocytes seem unchanged. The marginal glia are also definitely shrunken following urea.
In patients with cerebral hemisphere gliomas one finds large perivascular spaces in the cortex, apparently caused by the rupture of swollen astrocytic processes (brain oedema). After urea such perivascular exudates are significantly less common and smaller.
Definite changes in the white matter are not encountered. This is apparently the consequence of the paucity of vessels and of glial cells in this region.

Description des recherches, par la microscopie en contraste de phase et la microscopie électronique sur des modifications morphologiques du cerveau humain après injection intraveineuse d'urée, basées sur la biopsie de 18 malades.
Au microscope en contraste de phase, il apparaît que le cortex, après injection d'urée, devient nettement plus compact. Ceci est dû, comme le montre la microscopie électronique, à une déhydratation des cellules gliales et de leurs prolongements, l'oligodendroglie étant particulièrement affectée. Les cellules oligodendrogliales sont plus denses et plus petites après urée, tandis que les astrocytes ne paraissent pas modifiés. La glie marginale est aussi nettement diminuée de volume après urée.
Chez les malades ayant des gliomes hémisphériques on trouve de grands espaces périvasculaires dans le cortex, apparemment dus à la rupture de prolongements astrocytaires gonflés par l'oedème cérébral. Après urée, de tels exsudats sont, fait significatif, moins nombreux et plus petits.
On ne note pas de modifications nettes de la substance blanche, conséquence apparente de sa pauvreté en vaisseaux et en cellules gliales.
Riassunto
Vengono descritte le alterazioni morfologiche e del tessuto cerebrale umano in 18 pazienti dopo somministrazione endovenosa di urea osservate con il microscopio a contrasto di fase e con il microscopio elettronico.
Al microscopio a contrasto di fase la corteccia cerebrale dopo urea appare più compatta che di norma. Questo dipende, come è mostrato dai preparati al microscopio elettronico da una proliferazione delle cellule gliali e dai loro prolungamenti ai quali sono frammisti molte cellule oligodendrogliali.
Lo cellule oligodendrogliali sono più dense e più piccole dopo urea, mentre l'astroglia appare immodificata. La glia marginale appare invece chiaramentre raggrinzita.
In pazienti portatori di gliomi cerebrali si trovano nella corteccia cerebrale grosse lacune perivascolari, verosimilmente originate dalla rottura di prolungamenti astrocitari rigonfiati (edema cerebrale). Dopo urea gli essudati perivascolari sono chiaramente più rari e di minor importanza.
Nella sostanza bianca non è stata trovata alcuna alterazione sicura. Questo è verosimilmente dovuto alla povertà di vasi e di glia.

Descripción de las investigaciones con el microscopio de fase y la microscopia electrónica sobre las modificaciones morfológicas del cerebro humano después de la inyección intravenosa de urea, basadas sobre la biopsia de 18 enfermos.
Al microscopio en contraste de fase se encuentra que el cortex después de la inyección de urea se vuelve netamente más compacto. Esto es debido, como lo demuestra el microscopio electrónico, a una deshidratación de células gliales y de sus prolongaciones, la oligodendroglía se encontraba particularmente afectada. Las células oligodendrogliales son más densas y más pequeñas después de la urea, mientras que los astrocitos no parecian modificarse. La glia marginal está también netamente disminuida de volumen después de la urea.
En los enfermos con gliomas hemisféricos se encuentran grandes espacios perivasculares en el cortex, debidos aparentemente a la ruptura de prolongaciones astrocitarias hinchadas por el edema cerebral. Después de la urea es un hecho significativo que tales exudados son menos numerosos y más pequeños.
No se anotaron modificaciones netas en la sustancia blanca, consecuencia aparente de su pobreza en vasos y en células gliales.
