Zusammenfassung
Die Feinstruktur der Bruchschen Membran wird am hinteren Augenpol, insonderheit im Maculabereich, nach perforierenden Verletzungen, beim absoluten Glaukom und bei der Retinopathia diabetica untersucht. Ausgeprägte Veränderungen bestehen im Aufbau der Membran bei diesen Erkrankungen. Die kollagenen Faserschichten sind von Bläschen, elektronendichten Partikeln, zugrunde gehenden Fibrillen und von wabenförmigen Gebilden durchsetzt. Auch der elastische Gewebszug kann ebenso wie die Basalmembran der Pigmentepithel- und Endothelzellen mit in den Umbauprozeß einbezogen sein. Drusen finden sich nach längere Zeit zurückliegenden Erkrankungen vorwiegend in der inneren, kollagenen Faserschicht der Bruchschen Membran.
Die Untersuchungen zeigen, daß bei den einzelnen Krankheitsbildern die Bruchsche Membran und ihre angrenzenden Schichten unterschiedlich beteiligt sind. Die erhobenen Befunde deuten ferner darauf hin, daß die morphologischen Substrate in der Bruchschen Membran und in den Basalmembranen von der Art und Dauer der Erkrankung sowie vom Lebensalter der Patienten abhängig sind. Als krankheitsspezifische Zeichen können diese Veränderungen nicht aufgefaßt werden, da sie auch im hohen Alter ohne äußere Krankheitsursachen anzutreffen sind.
Summary
The fine structure of Bruch's membrane in the region of the posterior pole of the eye, particularly in the macular region, has been studied in eyes with perforating injuries, with absolute glaucoma and with diabetic retinopathy.
Considerable changes in structure of Bruch's membrane in these conditions are observable. Between the collagen fibers one can find vesicles, electron dense particles, altered fibrils and honeycomb like formations. Also the elastic tissue and the basement membrane of the pigment epithelium and of the endothelial cells show pathological structures. Drusen exist primarily in the inner collagenous zone.
The studies show that Bruch's membrane and the adjoining tissue undergo different alterations in structures in each of the above described conditions. Furthermore the results demonstrate that the pathologic structures in Bruch's membrane and in the basement membrane depend on the type and duration of the disease and on the age of the patients. These changes can not be interpreted as a specific sign of a particular pathologic process, since they are also observed in old patients who otherwise have no evidence of any disease.
